Clinical Case Manager Full Time / Fixed term - Surrey Hills

We are seeking a passionate Clinical Case Manager to join our team. In this full time, 12 month contract role, you will provide coordination of client services, including clinical support needs and provide support and guidance to support workers.

What You Will Be Doing

• Ensuring that client health and well-being, capabilities, independence, choice, privacy, dignity, and safety are maintained and enhanced through the provision of quality clinical care plan development and review.
• Assessment & development of care plans for new clients including those from Complex Care Pathway care plans.
• Supporting individuals to formulate goals and aspirations within the Peace of Mind Framework.
• Maintaining a current knowledge and best practice in clinical care and lifestyle service delivery.
• Striving to improve the quality of service provided within ONCALL | SACARE. Acting as a positive role model to nursing and support staff.
• Ensuring that all incidents are reported promptly and accurately with the appropriate risk assessment procedures undertaken.
• Ensuring that all relevant legislative requirements relating to care delivery are always complied with and ensures facility staff have awareness of those requirements.
• Maintaining a current knowledge of the NDIS High Intensity Daily Personal Activities requirements.
• Liaising with the Community Services Manager and Clinical Nurse - Learning & Development as it relates to decisions affecting client care & clinical needs.
  
  

From You, We Need

• Registered Nurse registration with the Australian Health Practitioner Regulation Agency (AHPRA)
• Manual handling certificate
• First Aid certificate
• NDIS Workers Check
• Working with Children's Check
• Clinical expertise in the management of medical conditions related to disability care
• Experience working in disability care
• Excellent administrative, time management and organisational skills
• Demonstrated positive and flexible attitude with a willingness to take on a wide range of diverse tasks and adjust to competing priorities
• Excellent interpersonal and communication skills with the ability to work with individuals from all socio-economic and cultural backgrounds
• Sound ability to use the Microsoft Office suite of products
