Growing together The Clinical Program Director for Heart Failure & Cardiovascular Programs is responsible for ideating, designing, and developing innovative technology-enabled specialty clinical programs focused initially on improving affordability, health outcomes, and member and provider experience for individuals with heart failure and closely related cardiovascular and cardiometabolic conditions. This role provides clinical leadership and oversight for the development and implementation of care pathways that support rapid initiation and optimization of guideline-directed medical therapy (GDMT) for heart failure. Recognizing that individuals with heart failure frequently have interconnected cardiovascular and cardiometabolic conditions, the Director will apply this expertise to related care pathways and, as organizational priorities evolve, contribute to the broader portfolio of cardiovascular program development, including coronary artery disease, arrhythmia management, cardiovascular devices, vascular disease, stroke, and aneurysm care. The Director works with healthcare analytics, pharmacy, clinical implementation, case management, technology, and medical management teams to develop, monitor, and continuously improve Cardiovascular Health Clinical Programs at UnitedHealthcare. The Director serves as a clinical subject matter expert, providing consultation on complex cases, evaluating program performance and outcomes, and identifying opportunities to optimize clinical pathways, improve operational efficiency, and enhance affordability and quality outcomes. The Director reports to the National Medical Director of Cardiovascular Health at UnitedHealthcare Clinical Services. The ideal candidate combines deep heart failure clinical expertise, broad cardiovascular knowledge, strategic thinking, an innovative mindset, program development capabilities, operational leadership experience, adaptability, and data-driven decision-making skills to create impactful solutions for members and providers. You'll enjoy the flexibility to work remotely * from anywhere within the U.S. as you take on some tough challenges. Primary Responsibilities: Clinical Program Strategy and Design Partner with the National Medical Director of Cardiovascular Health to identify opportunities and design scalable cardiovascular clinical programs that improve affordability, quality, clinical outcomes, and member and provider experience Lead the development of technology-enabled care models focused on heart failure, GDMT optimization, transitions of care, medication adherence, and coordinated longitudinal management Design clinical pathways, protocols, provider engagement strategies, decision-support tools, and operating models informed by evidence-based guidelines and real-world clinical practice Develop business cases, operational models, and implementation strategies that assess clinical impact, scalability, operational readiness, and financial performance Expand program development efforts across additional cardiovascular priorities, including arrhythmia management, coronary artery disease, vascular disease, stroke, cardiovascular devices, and related cardiometabolic conditions Clinical Leadership and Consultation Serve as the clinical subject matter expert for heart failure and cardiovascular program strategy, care pathway development, and GDMT optimization Provide consultation on complex cardiovascular cases and support appropriate escalation, provider engagement, and clinical decision-making Partner with clinical leaders to establish and maintain clinical governance processes, safety standards, escalation pathways, and care delivery expectations Support continuous evaluation of clinical trends, protocol performance, quality outcomes, and provider feedback to improve program effectiveness Data, Analytics and Performance Improvement Leverage clinical, claims, pharmacy, quality, utilization, and operational data to identify opportunities for program development and optimization Partner with analytics teams to develop KPIs, performance dashboards, and outcome measurement strategies Evaluate program performance, identify care variation and optimization opportunities, and recommend data-driven improvements Measure and communicate clinical, operational, quality, and affordability outcomes to support ongoing program refinement Cross-Functional Leadership and Stakeholder Engagement Lead and influence cross-functional partnerships across clinical, operational, pharmacy, analytics, technology, implementation, provider, and market teams Build alignment among executive leadership, provider organizations, and key stakeholders to advance cardiovascular program strategy and execution Collaborate with Regulatory, Compliance, Legal, and Clinical Governance partners to ensure compliant and sustainable program design Mentor and support clinical and non-clinical partners on cardiovascular care pathways, workflows, and performance improvement initiatives Program Execution and Continuous Improvement Support program implementation, transition planning, operational readiness, and adoption across business and clinical teams Monitor program performance and recommend enhancements that improve clinical outcomes, care coordination, member engagement, provider experience, and affordability Identify opportunities to increase scalability, efficiency, and innovation across cardiovascular and cardiometabolic care programs Support a culture of accountability, collaboration, clinical excellence, and continuous improvement You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in. Required Qualifications: 5+ years of clinical experience in heart failure, advanced cardiovascular care, or a related cardiovascular specialty Solid healthcare analytics experience utilizing claims, clinical, pharmacy, quality, and operational data to drive program performance and outcome measurement Experience designing, implementing, scaling, and optimizing cardiovascular, population health, value-based care, or technology-enabled clinical programs Experience developing KPIs, interpreting dashboards, and translating data into actionable clinical and operational strategies Experience collaborating with physicians, advanced practice clinicians, pharmacists, care management teams, provider organizations, and business partners to improve care delivery and outcomes Experience leading heart failure clinics, disease management programs, or advanced cardiovascular services Deep clinical expertise in heart failure, cardiovascular disease, GDMT optimization, evidence-based guidelines, and associated cardiometabolic conditions Familiarity with health plan regulatory, accreditation, compliance, privacy, documentation, and clinical governance requirements Proven success improving quality, utilization, affordability, operational performance, and patient outcomes through clinical program leadership Proven ability to lead and influence cross-functional teams in a matrixed healthcare environment Demonstrated ability to manage ambiguity, competing priorities, and complex initiatives while driving measurable results Demonstrated exceptional communication, presentation, negotiation, and stakeholder management skills Preferred Qualifications: Experience within payer, value-based care, accountable care, population health, or technology-enabled care environment Experience applying AI-enabled analytics, machine learning tools, clinical decision-support tools, or large language model capabilities in healthcare program design, clinical operations, or outcome measurement *All employees working remotely will be required to adhere to UnitedHealth Group's Telecommuter Policy Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $134,600 - $230,800 annually based on full-time employment. We comply with all minimum wage laws as applicable.
