Clinical Specailty Pharmacist

• Location: Remote
• Department: Ascension Pharmacy Services
• Schedule: Day shift | Full-time Monday-Friday| 8:00 AM - 4:30 PM

• Provide advanced clinical pharmacy care including counseling and ongoing pharmacotherapy management for patients with complex specialty conditions.
• Perform comprehensive medication therapy management (MTM) to identify drug interactions and optimize therapeutic outcomes.
• Develop and monitor individualized care plans, tracking laboratory values, clinical progression, and adverse drug events.
• Provide evidence-based clinical recommendations to healthcare providers regarding drug selection, dosing adjustments, and patient safety strategies.
• Document in the electronic health record (EHR) and specialty pharmacy software to meet accreditation standards.
• Collaborate across interprofessional teams to streamline prior authorizations, medication delivery logistics, and care transitions.

Licensure / Certification / Registration

• Required Credential(s):
  • Pharmacist obtained prior to hire date or job transfer date.

Education

• Bachelor's or advanced degree in Pharmacy required. Master's degree or Doctorate of Pharmacy preferred.

Work Experience

• None required.

Additional Preferences:

• Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy from an accredited institution.
• Active, unrestricted State Pharmacist License or licensure by date of hire.
• ASHP-Accredited PGY-1 Pharmacy residency or three (3) years of relevant clinical pharmacy experience required. ASHP-Accredited PGY-2 Pharmacy residency with a focus in ambulatory care or specialty pharmacy preferred.

• Active Tennessee license required by start date; partial remote role supporting multiple specialties in the Ascension Tennessee market including Gastroenterology and Pulmonology in Murfreesboro and New Salem.
