Schedule: Day | Full-Time
- Coordinate participant appointments and services, including care provided outside of the PACE center.
- Work with external vendors, providers, and community partners to schedule and coordinate participant services.
- Communicate with participants, families, and the care team to keep appointments and services on track.
- Help ensure participants have access to the care, transportation, and support services they need.
Education:
- High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.
- Note: Required professional licensure/certification can be used in lieu of education or experience, if applicable
