Clinical Supervisor/Case Manager RN

Job description:

The Clinical Supervisor supervises all clinical nursing staff (RNs, LVNs, HHAs) and provides training, mentoring and coaching as necessary.

Qualifications: Registered Nurse (RN) with current and unencumbered license to practice professional nursing in the State of Texas

Responsibilities: As a Clinical Supervisor/Case Manager RN, you will be responsible for overseeing patient care and ensuring high-quality service delivery within the home health setting. Reporting to the Director of Nursing, your role will involve managing active care plans, overseeing patient care delivery, educating/mentoring the patient care teams, and facilitating discharge planning with high quality outcomes. The clinical supervisor is also responsible for ensuring patient satisfaction, and responding timely to patient/employee reported problems/complaints. The clinical supervisor participates in the QAPI process as part of the QAPI committee, tracks infections and incidents for the assigned caseload.
