This position supports Bookmark Medical’s mission through adhering to and perpetuating coding best practices across the clinic organization. This role will be responsible for varying levels of risk adjustment, regulatory compliance and HCC diagnoses. What You Will Do Review medical record information on both a retroactive and prospective basis to identify, assess, monitor and document claims and encounter coding information as it pertains to risk adjustment Ensure that the diagnosis codes for each chronic or major medical condition have been captured and submitted within the permitted timeframe Assess adequacy of documentation of claims and query outpatient provider claims to obtain additional medical record documentation or to clarify documentation to ensure accurate and appropriate coding Audit provider documentation of ICD-10 codes and quality metrics to ensure adherence with CMS guidelines Demonstrates the ability to appropriately use coding principles to code to the highest specificity and comply with CMS regulations and company goals and policies Safeguards medical records and preserves the confidentiality of personal health information through the observance of physician network policies pertinent to the release of medical record information, record retention, and HIPAA privacy and security. What You Bring A minimum of 3 years of experience in advanced professional coding; HCC coding experience required. CMS and HHS preferred. Proficient with payer policies and guidelines Familiarity with Electronic Health Records (Athena, Epic, ECW) Strong communication skills Technology-savvy (Microsoft Office, Excel, Outlook) Education High School Diploma/Equivalent Required, Bachelor’s degree preferred Professional Coding Certification such as CPC, CCS, or CCS-P required; CRC preferred Why You Will Love Working Here We take care of our team so they can take care of patients.
