The Community Liaison-Homeless Population builds and supports relationships that assist persons living in homelessness to access needed services and resources. Working in collaboration with partner organizations this position will help facilitate linkage and system navigation to healthcare services. This position will also help build a system of care between healthcare, housing and homeless service systems by addressing system and service gaps. Position will assist with program development, consultation, resource management, and coordination of other services and agencies to best meet the needs of the persons served. This position is a provisional appointment that is tied to recurring funding. The extension of the position past June 2027 is dependent on ongoing funding. This is a full-time hybrid opportunity. There is no expectation of coming into the office routinely, however, the selected candidate must be available to report onsite to the Alliance Office for business meetings as needed and will be required to travel weekly throughout Wake County community 2-4 days per week to meet with members and providers. Responsibilities & Duties Access to Care Increase access to care for persons eligible for Alliance coverage who are identified as living in homelessness. For those not eligible for Alliance coverage will assist partner agencies to identify standard plan coverage Facilitate linkage to services and resources with community partners Will have on-site presence at key agencies where Alliance members regularly interact to increase engagement and access to care Coordinate follow up linkages and access to care for persons identified as high-cost users Community Impact and Partnerships Establish and grow collaborative relationship with County government and identified county safety net programs to help meet identified needs within the community Assist in coordinating healthcare partnerships and efforts for encampment outreach and decommissioning of encampments Provide intensive accompaniment to increase engagement as needed for persons identified in encampments Support all activities of System of Care Develop in-depth knowledge of various community systems and provide consultation to Care Management entities regarding available resources and barriers for individuals who are involved within homeless community systems, i.e., HMIS, Wake County Continuum of Care (CoC) Communication and Collaboration Conduct outreach and engagement with county entities Provide administrative and logistical support for special events, System of Care and HCL program activities Develop and maintain positive relationships with Alliance’s Supportive Housing team and care management teams serving Alliance members to ensure effective communication and coordination for persons living in homelessness Refer to Community Inclusion planning when appropriate to maximize member experience and to build social capital in member’s community of choice Participate in key coalitions and partnership spaces Serve as a subject matter expert to improve the knowledge and awareness of resources within the Wake County homeless service system for Alliance staff and providers Implement Program Activities Ensure timely documentation of tracking tools and other activities as required by program Analyze and monitor community capacity for service needs, service gaps, and the implementation of evidence based/best practices Assist with the development of community level data for evaluation and planning Minimum Requirements Education & Experience Bachelor’s degree in a Human Services related field from an accredited college/university and three (3) years of experience working with homeless populations and individuals facing complex challenges or three (3) years of progressive experience in the field of Health Care or Human Services. Previous Wake County Homeless system experience. Permanent resident in Wake County or surrounding counties. Knowledge, Skills, & Abilities Knowledge of the Homeless Management Information System (HMIS) and Continuum of care (CoC) Ability to establish and maintain favorable working relationships with individuals, community stakeholders and government employees is necessary Ability to build rapport and trust with homeless individuals Demonstrated exceptional interpersonal skills, highly effective communication ability, and the propensity to make prompt independent decisions based upon relevant facts Ability to make prompt independent decisions based upon relevant facts and demonstrated high level of diplomacy and discretion to effectively negotiate and resolve issues Demonstrated knowledge of the primary purpose and functions of community agencies Employment for this position is contingent upon a satisfactory background and MVR (Motor Vehicle Registration) check, which will be performed after acceptance of an offer of employment and prior to the employee's start date.
