The Diversion Outreach Specialist provides an essential function of the Transition to Community Living (TCL) program by supporting members with information about, and assistance with, accessing services and community resources to improve the individual’s health outcomes and aid in successful community living. The Outreach Specialist also supports MCO efforts through a combination of activities with various public and private agencies, development of processes and agreements to address critical community needs of individuals participating in TCL , foster relationships and partnerships to aid in the identification and referral to Alliance of individuals that meet special needs or high-risk criteria. This position is also responsible for administering key assessments to help determine community needs and working in collaboration with several internal teams.
This is a full-time hybrid opportunity. There is no expectation of coming into the office routinely, however, the selected candidate must be available to report onsite to the Alliance Office (Charlotte, North Carolina) for business meetings as needed. The successful candidate will also be required to travel as needed throughout Mecklenburg County to meet with members, providers and/or other community stakeholders.
Responsibilities & Duties
Conduct Assessments and Planning
- Assist Diversion Clinician (Licensed Clinician) with members transitioning to the community from institutional care settings to community-based care
- Assist Diversion Clinician in essential functions related to screening RSVP (Referral Screening Verification Process) submissions in a timely manner
- Utilize person centered planning, motivational interviewing and assessments to review information and develop rapport with the members supported
- Obtain necessary releases of information that will improve care management activities on behalf of the member
- Provide education and supports to members and legal guardians regarding their rights and responsibilities, available service options, providers availability, and payer requirements that may impact service connection and maintenance
- Actively collaborate with members supported and members of the planning team to ensure development of a plan that accurately reflects the individual’s needs and desired life goals
- Ensure that assessments and plans are updated, as needed, whenever the members’ life circumstances change
- Educate members, guardians and families about provider choice, person-centered planning, using cultural competence and available local resources
- Explain the benefits and financial aspects of Supported Housing, as well as other clinically appropriate community based integrated settings
- Ensure members receive information and education about choice of services, supports, housing and community-based resources including the advantages or disadvantages
Conduct Monitoring/Coordination
- Notify Diversion Clinician, Supervisor and/or Care Manager of any new service needs identified during service monitoring
- Attend all required meetings related to care planning and provider engagement
- Escalate high risk/high visibility and/or complex barriers/needs members who may have SDOH/Behavioral/Physical needs to Diversion Clinician for additional supports
- Promote customer satisfaction through ongoing communication and timely follow-up on any concerns/issues
- Verify members Medicaid and promptly follow up on identified issues
Maintain Documentation
- Ensures all documentation (e.g. goals, plans, progress notes, etc.) meet state, organization, and Medicaid requirements
- Document in all relevant databases in a timely manner
- Monitor documentation to ensure that issue/errors are resolved
- Follow administrative procedures and effectively manage caseload
Data analysis
- Utilize data/risk stratification to analyze needs of the members we serve, identify resource needs and consistency of workflow implementation across Community Health and Well-being
Minimum Requirements
Education & Experience
Bachelor’s degree in a Human Services field and five (5) years of experience with the population served (SED/SMI or IDD/TBI)
Or
Bachelor’s-level RN plus three (3) year of relevant experience working directly with individuals with SMI.
Or
Master’s degree in a human services field and three (3) years of experience with the population served (SED/SMI or IDD/TBI)
Preferred
Master’s degree in human services preferred.
2 years of Full Time, Post degree work experience with social service agencies.
Knowledge, Skills, & Abilities
- Knowledge of resources and systems in the community that can assist with eliminating SDOH barriers to treatment and whole person living.
- A high level of diplomacy and discretion is required
- Problem solving, negotiation, arbitration and conflict resolution skills
- Must be highly skilled at shifting between macro and micro level planning
- Detail oriented
- Ability to organize multiple tasks and priorities, and to effectively manage projects from start to finish.
- Work activities and quickly adapt to mandated changes and priorities within the department.
- The ability to change the focus of his/her activities to meet changing priorities.
- Proficiency in Microsoft Office products (such as Word, Excel, Outlook, PowerPoint, etc.) is required.
Salary Range
$54,631 - $69,655/ Annual
Exact compensation will be determined based on the candidate's education, experience, external market data and consideration of internal equity
