Position: Field Outreach Representative

The Opportunity:

This is a field-based, performance-driven role for someone who thrives on independence, loves building rapport quickly, and wants their work to mean something beyond a number. You will be out in the community every day, meeting eligible patients at home, earning their trust, and helping them take a step that could genuinely improve their health. Healthcare experience is not required, and we will provide training and support to help you succeed.

What You’ll Do

  • Visit eligible patients in their homes and introduce them to the Monogram Health program
  • Build trust through respectful, face-to-face conversations, listen to patient needs, answer questions, and help eligible patients feel comfortable enrolling
  • Work toward clear daily expectations for patient visits, meaningful conversations, completed enrollments, and other performance goals
  • Route efficiently across assigned patients, complete scheduled visits, and conduct consistent follow-up to support patient engagement and enrollment goals.
  • Keep accurate CRM records, track follow-up activity, and stay accountable to monthly individual performance goals

What You’ll Need

  • Required: High school diploma or GED
  • Valid driver’s license, reliable transportation, and willingness to travel throughout the assigned territory each day
  • Comfort with high-volume, in-person, door-to-door outreach, this is a field role
  • Strong communication, relationship-building, organization, and follow-up skills
  • Self-motivation, resilience, adaptability, and comfort working independently toward measurable goals
  • Nice to Have: Bachelor’s degree or experience in field outreach, community engagement, enrollment, business development, account management, customer service, or another goal-oriented role involving direct customer interaction
  • Comfort using CRM tools to document interactions, organize follow-up activity, and track progress

Compensation

The base salary range for this role is $55,000 to $72,000.

Target Total Compensation Opportunity: $65,000 - $130,000+

Annual Incentive Opportunity: Incentive earnings are uncapped and are paid in addition to base salary. While earnings vary based on individual performance, most team members typically earn approximately $25,000 to $65,000 annually in incentive compensation, with top performers having the opportunity to earn significantly more.

Final compensation will be determined based on geographic location. While a larger base salary range is posted, most offers will typically fall within a narrower band based on these factors.

In addition to base salary, this role is eligible for additional compensation, including bonus opportunities and a comprehensive benefits package.
