Finger Lakes Community Health (FLCH) was founded in 1989 with an original mission of serving the region's agricultural workers by linking individuals to area resources and providing essential health services. In 2009 the organization expanded to become a Federally Qualified Health Center (FQHC), serving patients of all incomes, ethnicities, and walks of life. Over the last decade, FLCH has grown to provide medical, dental, and behavioral health services to over 28,000 patients at eight health centers. Position Summary
The Health Homes of Upstate NY(HHUNY) Coordinator oversees HHUNY and Geriatric Care Manager staff to ensure patients receive timely, supportive, and culturally appropriate assistance when accessing healthcare services in accordance with Health Home core services. This role manages daily operations, staff scheduling, workflow efficiency, and the quality of patient support services while addressing barriers to care and facilitating smooth access to medical, dental, and behavioral health services. This role will also carry a caseload consisting of a total of up to 20 patients (following High, Medium, Low (HML) billing standards)
Main Responsibilities
Operations
- Ensure staff provide quality Health Home core services:
- comprehensive care management,
- care coordination and health promotion,
- comprehensive transitional care,
- individual and family support,
- referrals to community and social support services,
- use of Health Information Technology to link services.
- Ensure staff provide accurate information, patient education, and assistance accessing programs and services.
- Address complex or escalated patient concerns and coordinate solutions with clinical or operational teams.
- Oversee appointment coordination, referrals, and follow-up workflows to support continuity of care.
- Coordinate patient transportation services to support access to care.
- Partner with internal teams or external vendors to address transportation needs when necessary.
Workflow & Quality Management
- Maintain consistent Care Management workflows, patient education processes, and documentation standards.
- Ensure timely and accurate documentation in the electronic health record (EHR).
- Monitor departmental performance metrics and support quality improvement initiatives.
- Identify workflow gaps and assist in developing and implementing improved processes.
- Ensure staff adherence to organizational policies, procedures, and service standards.
Collaboration & Communication
- Partner with clinical teams, call center staff, and outreach programs to improve patient access.
- Participate in meetings related to care coordination and operational improvement.
- Serve as a resource to staff regarding patient access barriers and navigation best practices.
Staff Supervision
- Supervise the performance of HHUNY and Geriatric Care Manager staff, including conducting evaluations, auditing documentation and data entry, providing counseling, training, and administering disciplinary actions.
- Ensure staff are trained and performing in alignment with departmental standards.
- Coordinate schedules to ensure coverage and support efficient patient flow.
- Provide HHUNY Care Management services to up to 20 patients, following the billing standards, to foster a supportive team environment.

Work Environment
- Health Center/Clinic: Work within a healthcare facility, interacting with patients and staff.
- Outreach: Outreach work within community and field setting, engaging with underserved and at-risk communities.
- Remote: Home or remote office, using virtual communication to collaborate and interact with patients and staff.
- Moderate-Paced: Dynamic environment that requires both productivity and accuracy to reach goals and project objectives.
- Office Hours: Standard business hours, with occasional extended shifts based on clinic needs.
- On-Call Hours: Outside of standard office hours, including evenings, weekends, and holidays to respond to urgent needs. Schedule determined by department needs.
- OSHA Level II
Education
- Bachelor’s degree in a health or human services field plus two years of relevant experience OR a Master’s degree in a related field with one year of experience OR equivalent experience in the field.
- Health Home of Upstate New York (HHUNY) Plus certification required.
Qualifications
- Screening and clearance through the NYS Justice Center of Staff Exclusion List and criminal background check
- 3+ years in a healthcare or customer service setting, functioning at an independent level with a solid foundation of role knowledge
- Prior experience in a supervisory or administrative position preferred.
- A valid government-issued photo identification document is required.
- A valid New York State driver’s license in compliance with FLCH driver’s policy is required.
- Current New York State automobile insurance in compliance with FLCH underwriting agreement is required.
- Policy limits should reflect $100,000/$300,000/$100,000 coverage.
- Serves as a resource and educates on effective outreach and support programs.
- Has advanced expertise in facilitating team communication and conflict resolution.
- Applies advanced reasoning in complex situations to bring solutions to team.
- Applies advanced expertise to identify complex and recurring challenges.
