SHIELDS’ mission is to develop, deliver and evaluate culturally sensitive, comprehensive service models that empower and advocate for high-risk families in South Los Angeles. We are seeking a compassionate and motivated In-Home Outreach Counselor to provide direct outreach, counseling, and case management services to individuals and families in the community. The ideal candidate will engage residents through in-home outreach and coordinate services that promote stability, independence, and overall well-being. Duties Conduct in-home outreach services, including distributing literature, door-to-door canvassing, and engaging residents through phone and in-person contact. Target and engage existing community groups and activities within residential developments. Provide in-home counseling services to clients and their families in accordance with the appropriate level of care. Coordinate and provide support services that assist clients with independent living skills. Deliver comprehensive case management services, including linking clients to auxiliary services and assisting with access to community resources. Assess client and family needs related to social, emotional, financial, and medical challenges, and coordinate appropriate service delivery. Collaborate and maintain communication with referral sources such as the Department of Children and Family Services (DCFS) and the Department of Probation. Maintain positive and professional working relationships with community partners, linkage agencies, and referral organizations. Stay informed of available community programs, services offered, and eligibility requirements. Maintain accurate and timely case documentation in compliance with agency policies and all applicable federal, state, county, and local regulations. Bachelor’s degree candidates must have a minimum of two (2) years of direct service experience in social services or a related field (DCFS approval required). Experience working with high-risk families and children, under-served populations, and/or substance abuse issues preferred. Demonstrated therapeutic, advocacy, and case management skills through education, training, or professional experience. Strong verbal and written communication skills, with the ability to present information clearly and effectively. Proficiency in basic computer skills, including word processing software. Ability to exercise sound judgment, analyze situations, and make independent decisions. Commitment to teamwork and agency values and philosophies. Ability to adapt to changing program needs and acquire new skills as needed. Culturally responsive and respectful approach when working with diverse communities and clients.
