Build meaningful connections across hospital and community care while helping people move forward with confidence, support, and continuity. This is a remote position open to Missouri Residents only. The Liaison Specialist plays a key role in connecting patients with the care, resources, and follow-up services that support their health and well-being. Working as part of an integrated care team with Compass Health and SSM Health, this role helps strengthen continuity of care across hospital, primary care, behavioral health, and community settings. The Liaison Specialist builds relationships, coordinates referrals, supports health literacy, and helps identify and address barriers to care—all while honoring the strengths, goals, and individual needs of each person served. This is a great opportunity for someone who enjoys building relationships, coordinating care across multiple systems, and helping people navigate healthcare and community resources. You’ll collaborate with hospital staff, healthcare providers, community organizations, and other partners to help create connected, person-centered pathways to care. Key Responsibilities Coordinate referrals and follow-up care across medical, dental, behavioral health, primary care, and community services. Follow up with patients after hospital or emergency department visits and support timely access to needed services. Assess access barriers such as transportation, housing, and economic stability, and connect patients with appropriate resources. Partner with patients to identify strengths, needs, goals, and next steps while providing support, education, advocacy, and follow-through. Promote wellness, health literacy, and self-management through culturally responsive education and encouragement. Build and maintain strong referral relationships with community organizations, schools, employers, healthcare providers, and other partners. Support care team communication, program tracking, reporting, and documentation to promote effective outcomes. Contribute to a collaborative, respectful, and welcoming environment for patients, families, visitors, and colleagues. Maintain accurate, timely documentation and information systems in accordance with organizational policies and procedures. Requirements, Skills, Knowledge and Expertise • High School Graduate or GED required • One (1) to Two (2) years of relevant work experience required • Familiar with medical terminology, insurance coverage, and inpatient/outpatient hospital services • Must be proficient in Microsoft Office applications, including Word, Excel, and Outlook, as required for the role You’ll be a great fit for this role if you: Build trusting relationships and communicate with warmth, respect, and professionalism.
