Licensed Clinical Social Worker

• Location: Remote/Chicagoland Area

• Facility: Ascension Illinois

• Department: Case Management

• Schedule: Day shift | Full-time

• Coordinate the interdisciplinary plan of care across the continuum from admission to discharge while integrating patient and family preferences

• Monitor patient care through assessments, evaluation, and record reviews while serving as a resource for documentation, service appropriateness, and post-discharge options

• Advocate for necessary resources and barrier removal while maintaining ongoing dialogue with supervisors and providers to execute care plans

• Provide clinical oversight of other associates as needed

Licensure / Certification / Registration:

• One or more of the following required:
  • Clinical Counselor obtained prior to hire date or job transfer date.
  • Counselor specializing in Clinical Practice obtained prior to hire date or job transfer date.
  • Social Worker specializing in Clinical Practice obtained prior to hire date or job transfer date.
• Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
• Case Manager credentialed from the American Case Management Association (ACMA) preferred.

Education:

• Master's degree in Social Work required.

Work Experience:

• 3 years of experience preferred
• LCSW in IL
• Hospital care management experience needed
• Previous Behavioral Health experience desired
• Able to commute, if needed, to the Chicago, IL area
