Licensed Practical Nurse - Care Coordinator

The Location: Kalele Care Services was founded in 2020 by Ohana Pacific Health to address the complex healthcare needs of the community with an emphasis on assisting Kupuna to live longer and healthier independent lives. This remote/hybrid position will be based on Oahu. The Position: The Licensed Practical Nurse (LPN) – Care Coordinator serves a critical role in an integrated care management model, delivering clinical care coordination and telephonic Chronic Care Management (CCM) under general practitioner supervision. Telephonic Patient Encounters: Conduct structured telephone interactions with enrolled CCM patients to assess health status, symptom control, and care plan progress. Care Plan Maintenance: Assist in updating and maintaining comprehensive, patient-centered electronic care plans in the EHR following CMS clinical guidelines. Non-Face-to-Face Time Tracking: Document all eligible non-face-to-face clinical care management time to support CCM billing requirements. Medication Adherence & Review: Perform routine telephonic medication reviews, identify gaps in adherence or refill barriers, offer patient education, and escalate complex drug-related problems to the supervising provider. Symptom Monitoring & Escalation: Monitor patient-reported symptoms and vitals via telephone; identify early signs of clinical decompensation and escalate to treating providers as appropriate. Requirements: Current State of Hawaii Licensed Practical Nurse (LPN) licensure in good standing. Proficiency in basic computer skills including but not limited to, navigating web browsers, electronic medical record systems, computer systems and applications, Microsoft office suite Experience in healthcare or home health field or work with the geriatric population, preferred.
