Remote Behavioral Health Coordinator (BHC) – Post-Acute Care Reports To: Behavioral Health Case Manager (BHCM) Position Summary The Remote Behavioral Health Coordinator (BHC) supports Psych Health Associates' (PHA) Behavioral Health Integration (BHI) program across assigned post-acute facilities. The role is fully remote and open only to candidates who hold a current Medical Assistant (MA) certification, a Licensed Practical Nurse (LPN) license, or a Registered Nurse (RN) license. The BHC's main job is to manage the front end of the program. That means monitoring facility census, confirming patient eligibility, completing enrollment, and getting and documenting consent through the EHR or by contacting the patient's Power of Attorney (POA) or responsible party. The BHC also covers facilities remotely when the in-person BHI Coordinator is on PTO, and takes on overage when monthly caseload is more than in-person staff can handle. This is a full-time position with regularly scheduled hours set by PHA. Assignments, facility coverage, and caseload may shift with census volume, in-person coordinator PTO, monthly overage, and program capacity. The BHC is expected to be flexible within scheduled hours to meet coverage needs. This role requires strong organizational skills, professionalism, cultural competence, the ability to work independently, proficiency with EHRs and digital tools, and strict adherence to PHA policies, workflows, and documentation standards. Key Responsibilities Census Oversight, Eligibility & Enrollment Monitor facility census in the EHR for admissions, discharges, readmissions, transfers, and payer changes across assigned facilities. Screen new and current residents for BHI eligibility using PHA criteria, CMS requirements, and applicable value-based care (VBC) program guidelines. Enroll eligible patients in the BHI program and keep the enrollment roster accurate and current. Work with facility staff to resolve census, payer, or demographic discrepancies that affect eligibility. Report census trends, enrollment status, and missed or pending enrollments to the BHCM as required. Consent Get patient consent through the EHR or, when the patient cannot consent, by phone or digital outreach to the POA, legal guardian, or responsible party. Verify and document the legal authority of any surrogate decision-maker before getting consent on the patient's behalf. Document consent, outreach attempts, and outcomes accurately and on time, following PHA policy and CMS requirements. Track pending, declined, and revoked consents, and follow up within PHA timeframes. Remote Facility Coverage (PTO & Overage) Provide remote coverage for assigned facilities when the in-person BHI Coordinator is on PTO or otherwise unavailable. Support caseload overage for the month when volume is more than in-person staff can cover. During coverage, complete required monthly behavioral health screenings and documentation remotely, according to patient needs and PHA protocols. Identify changes in condition, safety concerns, or other clinical updates and escalate them promptly to the BHCM or the appropriate clinical team. Work within the scope of practice for your credential (MA, LPN, or RN) at all times. Facility Collaboration Act as a remote point of contact for BHI coordination at assigned facilities. Communicate with facility leadership, nursing, social services, therapy, business office/admissions, and IDT members by phone, Teams, and other approved channels. Help with scheduling and logistics for psychiatric and psychology services when needed. Documentation & Compliance Complete all required documentation accurately and on time, according to PHA policies and workflows. Maintain eligibility, enrollment, consent, screening, and communication records in the EHR. Adhere to HIPAA, privacy, compliance, and behavioral health workflow standards. Internal Operations, Escalation & Team Collaboration Work with the BHCM and in-person BHI Coordinators on handoffs, coverage schedules, caseload updates, and workflow execution. Accept facility, coverage, and overage assignments as directed by the BHCM, and adjust priorities to meet program needs. Take part in routine check-ins, supervision, and team meetings as assigned. Follow escalation pathways for clinically significant changes, facility concerns, workflow barriers, and safety-related observations. Qualifications Required One of the following, active and in good standing: Medical Assistant certification (e.g., CCMA, CMA, RMA) Florida LPN license, or a multistate compact license with Florida privilege Florida RN license, or a multistate compact license with Florida privilege Experience in healthcare, care coordination, case management, or a related patient-facing role. Proficiency with EHRs and digital communication tools. Ability to manage high-volume census review and caseload across multiple facilities. Strong communication, organization, critical thinking, and follow-through. Professional phone communication skills, including sensitive conversations with families and POAs. Demonstrated cultural competency and ability to work effectively with diverse populations. Preferred Experience in post-acute, skilled nursing, assisted living, or other long-term care settings. Experience with Medicare/Medicaid eligibility, enrollment, or consent workflows (e.g., BHI, CCM, or similar programs). Experience supporting behavioral health services. Experience with PointClickCare or similar long-term care EHRs. Bilingual (e.g., Spanish) preferred but not required. Remote Work Expectations, Equipment & Work Environment Work is performed 100% remotely using PHA-approved tools and platforms. No on-site facility presence is required. Work from a dedicated, private, distraction-minimized, HIPAA-compliant workspace. Maintain reliable high-speed internet, professional communication standards, and availability during scheduled hours. Frequent use of a laptop, smartphone, EHR, and digital communication tools (e.g., Microsoft Teams). Ability to sit for extended periods while working at a computer.
