Position Summary
This position provides care coordination services for the Division's programs in accordance with HFS, UIC and DSCC

Administrative Rules and Policies with supervisory assistance of the Assistant Director of Operations of Core/Connect

Care/Home Care, when appropriate. The successful candidate will receive technical training and support from Core/ConnectCare/Home Care Regional Managers, when appropriate as well as Central Administrative Office departments.

  • Provides care coordination services across all programs: Fills in for Care Coordinator vacancy across programs as needed. Facilitates the implementation of the care coordination plan. Monitors the care coordination plan activities. Revises the care coordination plan to address the changing and ongoing concerns and priorities of the family. Coordinates all available health care benefits. Participates as a member of the Regional Office multidisciplinary team. Complies with University, Division and Regional Office policy and procedures. Complies with state and federal regulations related to children enrolled in the Core, Connect Care and Home Care Programs.
  • Identifies children eligible for DSCC services: Assists in determining general eligibility for each assigned case along with Program Coordinator Assistant. Determines medical eligibility for each assigned case. Assists in the completion of the financial application process for each assigned case along with Program Coordinator Assistant. Enter Percent.
  • Enters, prepare, transmits, updates, scans, and uploads information/documents in electronic records: Demographic and medical social data from applicants/recipients, families including completing forms and letters in support of care coordination services. Receives, processes and routes incoming/outgoing written applicant/recipient correspondence, reports, etc.
  • Participates in special projects impacting DSCC: Participates in organizational committees/projects on a regional or statewide basis. Participates in DSCC sponsored interagency programs. Performs other duties as assigned. Review, advise, support equipment requests submitted to HFS Prior Approval Unit and returned by HFS Prior Approval unit for additional information.
  • Performs additional duties as assigned. Perform other related duties and participate in special projects as assigned.
  • Minimum Qualifications

      1. Bachelor's degree with a major in the social or behavioral science or related health specialty from an accredited college or university.
        OR
      2. Master's degree in social or behavioral science or related health specialty area from an accredited college or university.
    1. One (1) year (12 months) of progressively more responsible full-time experience in the practice of social services/ agencies or medical/clinical setting maintaining acceptable standards of social work practices


    Preferred Qualifications
    Bachelor’s degree in Nursing, Social Work, or other related field.
