We are seeking an experienced Nurse Practitioner to join a growing clinical team serving patients in Delaware. Through telehealth, remote care management, and close coordination with patients, families, caregivers, health plans, and primary care providers, the team focuses on prevention, early intervention, and better outcomes. Our patient population may include both Medicaid and Dual Eligible members, including individuals with: Complex chronic medical conditions Physical disabilities Traumatic brain injuries Intellectual and developmental disabilities Functional limitations Medically complex elderly populations. What You'll Be Doing: As a Nurse Practitioner, you will play an important role in managing and coordinating care for medically complex patients. Your responsibilities will include: Providing clinical care and ongoing management through telehealth and remote care technologies. Managing medically complex Medicaid and Dual Eligible patient populations. Collaborating with physicians and an interdisciplinary care team to develop and execute individualized care plans. Working closely with patients, families, caregivers, primary care providers, specialists, and health plan partners. Identifying changes in patient health and intervening early to help prevent unnecessary emergency department visits and hospitalizations. Supporting patients through hospital admissions, discharges, and transitions back home. Participating in advance care planning and goals-of-care discussions with patients and families. Participating in a shared clinical triage and support model. Supporting quality initiatives, including accurate documentation related to HCC coding, risk adjustment, and closure of quality gaps. Contributing to clinical best practices and helping shape a growing care model. Location & Licensure This position will serve patients in Delaware. Candidates who currently live and are licensed in Delaware or a bordering state will be considered. If you do not currently hold a Delaware license but are selected for the position, the organization is willing to support the Delaware licensure process and cover the cost of obtaining your Delaware license. Required qualifications include: Current Nurse Practitioner license in Delaware or an active NP license in a bordering state with the willingness to obtain Delaware licensure. The organization will cover the cost of obtaining Delaware licensure for the selected candidate, if needed. Family Nurse Practitioner (FNP) or Adult/Adult-Gerontology Nurse Practitioner certification required. At least 3–5 years of clinical experience as a Nurse Practitioner. Ability to practice autonomously with appropriate prescriptive authority. Experience caring for medically complex adult and/or geriatric populations. Experience with telehealth, virtual care, or remote patient management. Experience with or understanding of HCC coding and risk adjustment. Familiarity with quality measures, including STAR and HEDIS initiatives. Experience conducting advance care planning and goals-of-care discussions. Strong communication and relationship-building skills.
