*This is an on-call position that does NOT require travel. On-call hours are 5pm-8am, Monday-Friday, and all day Saturday and Sunday.
As the On-Call Triage RN, you'll use your nursing expertise to assess patient needs, provide education and support, coordinate care, and guide timely interventions that allow the patient to remain safe and comfortable at home. Every call is an opportunity to bring reassurance, clinical expertise, and peace of mind to patients and families during unexpected moments.

Your Impact
- Provide telephonic triage support for home health patients and caregivers during evenings, nights, weekends, and holidays
- Assess patient symptoms and clinical concerns, utilizing sound clinical judgment to determine appropriate interventions
- Provide guidance regarding symptom management, medications, treatment plans, and home care instructions
- Escalate urgent concerns and facilitate appropriate medical attention when necessary
- Coordinate communication between patients, caregivers, physicians, and the home health care team
- Obtain, document, and communicate physician orders as needed
- Accurately document patient interactions, assessments, recommendations, and care coordination activities in the electronic medical record
- Ensure continuity of care through detailed communication and handoff to daytime clinical staff
- Assist with after-hours patient inquiries and care coordination needs
- Maintain compliance with agency policies, regulatory requirements, and home health standards of practice
- Participate in quality improvement initiatives and ongoing clinical education
Requirements
- Currently licensed as a Registered Nurse (RN) in the State of Arizona
- 1 year of home health experience strongly preferred
- Previous triage, telehealth, on-call, or case management experience
- Knowledge of Medicare home health regulations and standards
- Experience supporting patients with chronic disease management, medication management, and complex care needs
