Develop individualised care plans in partnership with the client, their family and other healthcare professionals . Effectively communicate with staff on the clinical condition/diagnosis and treatment/discharge plans of clients and ensure accurate records are maintained by staff . Identify and manage risks as part of care provision and lead the investigation and resolution of clinical incidents, ensuring timely and thorough documentation. Support staff by delivering client-specific clinical training, mentorship and supervision by providing feedback, conducting performance reviews, and supporting professional development.
