Reports to: Patient Care Navigation Manager

Location: Remote


The Patient Care Navigator (PCN) provides a comprehensive, proactive, and “Patients First” experience for our patients and referring offices. The PCN engages the patient and referring provider when a referral is received and remains the primary point of contact for the duration of the admissions process. An effective PCN possesses excellent written and oral communication skills and the ability to work equally well with patients, physicians, corporate support staff and other health care providers involved in the patient’s care.


Primary Duties and Responsibilities:

  • Serves as the communication hub for the patient, referring provider, VIVO Clinical Operations, VIVO Intake Department, RISS Team and PRO Teams.
  • Establishes strong working relationships with assigned physicians/practice managers, RISS and VIVO support teams.
  • Understands and communicates the physician preferences for patient care.
  • Establishes rapport with patients and caregivers.
  • Professionally investigates, triages, and resolves patient issues, ensuring patients experience outstanding care. Proactively assists patients to minimize patient issues.
  • Escalates issues when identified to the appropriate party, e.g. physician, department supervisors, etc.
  • Coordinates with support teams to ensure accuracy and optimal patient experience across the VIVO continuum of care, e.g. intake registration, insurance verification, scheduling and PRO resources.
  • Evaluates needs and coordinates with education department to create content/experts for In-services to enhance team knowledge and performance.
  • Supports and participates in process improvement efforts.
  • Follows standards and guidelines for telephones etiquette and service when interacting with patients over the phone.
  • Supports and adheres to VIVO policies and procedures.

Qualifications:

This position requires the ability to build strong working relationships, solve problems and document interactions appropriately. The PCN ensures all parties involved receive accurate and timely responses to questions and concerns, as well as ensuring positive and efficient patient experiences while reducing financial loss and ensuring compliance.

Education:

  • High school diploma or equivalent, required

Experience:

  • Minimum 3 years of related prior work experience in a healthcare environment; outpatient clinic settings preferred.
  • Knowledge of medical terminology required.
  • Proven competencies can take precedence over prior work experience.
  • Prior experience working with sensitive information and maintaining confidentiality, required

Skills:

  • Proficient (intermediate level of application) with all Microsoft Office Products programs.
  • Must possess outstanding communication and interpersonal skills, be able to communicate professionally and effectively with other staff members, patients, vendors, and physicians, and is fluent in written and spoken English.
  • Technical ability to navigate a system for data input
  • Ability to perform basic math sufficient to understand patient financial obligations and concerns.
  • Working knowledge of PC window applications sufficient to input and retrieve data.
  • A desire and commitment to provide the highest level of satisfaction in quality service, caring and compassion to patients, their families and caregivers.
  • High level of customer service experience, exemplary interpersonal skills and ability to interact with patients and families in a wide variety of sensitive situations.
  • Easily adapts to changing technology, policies, procedures and processes.
  • Demonstrated interest in process improvement or adopting new processes and or technology.
  • Demonstrated ability to understand medical insurance information
  • Critical thinking, decisive judgment, and ability to work independently with minimal supervision in a hectic environment. Demonstrated success in working remotely.
  • Demonstrated independent thinking and problem-solving skills, ability to exercise judgment to triage issues and concerns.
  • Demonstrated ability to switch from one task to another successfully in a fast-paced, ever-changing environment.
