Job Title: Remote Clinical Pharmacist – Medicaid MTM Case Manager (Missouri License Required) Job Description This role focuses on providing compassionate, telehealth-based medication therapy management (MTM) and case management services to Medicaid patients in Missouri, many of whom are furthest from traditional healthcare opportunities. As a remote clinical pharmacist, you build ongoing relationships with patients through phone and telehealth, support their physical and mental health needs, and coordinate closely with providers, caregivers, and community resources to improve medication adherence, understanding, and overall health outcomes. Responsibilities Serve as a pharmacist case manager for Medicaid patients in Missouri, re-introducing them to the program and building rapport and trust through telehealth and phone-based interactions. Provide MTM-related services to Medicaid patients, including counseling participants on the importance of medication adherence and proactively addressing missed doses and refills. Deliver clear and accessible medication education, ensuring patients understand their therapies, dosing, potential side effects, and the importance of continued adherence. Provide self-care education for specific chronic conditions, helping patients manage their health more effectively between provider visits. Contact physicians to schedule necessary diagnostic testing and follow-up appointments related to medication therapy and chronic disease management. Contact physicians to discuss and optimize medication therapy, collaborating on treatment adjustments and supporting community health workers who advocate for participants. Connect participants with appropriate community-based resources as needed to support their health, social needs, and overall well-being. Assess each participant’s health status, including basic physical and mental health assessments, using tools such as DSM-based questionnaires and clinical indicators like glucose levels and blood pressure. Develop individualized medication treatment plans that align with existing diagnoses, lab results, and overall care goals. Monitor and evaluate participants’ responses to therapy, adjusting recommendations and plans as needed to improve outcomes. Provide comprehensive medication reviews to identify, resolve, and prevent medication-related problems and potential drug therapy issues. Document all care provided in a clear, accurate, and timely manner, and communicate essential information to participants’ primary care providers and other members of the care team. Offer oral education and training to enhance participant understanding and appropriate use of medications, tailored to patients with limited income and resources. Provide information, support services, and resources to strengthen participant adherence to therapeutic regimens and long-term care plans. Coordinate and integrate MTM services within each participant’s broader healthcare services, including caregivers, community health workers, case managers, and advocates. Arrange transportation services for participants who need to meet a provider in person, ensuring access to necessary care. Maintain thorough program documentation for each service provided, including patient identifiers, verification of services, dates, time spent in 15-minute increments, start and end times, and service setting. Record treatment plans, evaluations, tests, findings, results, and prescriptions as necessary, ensuring compliance with program documentation standards. Establish and maintain records containing each participant’s or responsible person’s signature for services furnished by the pharmacy. Coordinate with patients to schedule MTM-related services, including selecting appropriate days and times for telehealth or phone consultations. Obtain and coordinate patient signatures directly after services are rendered, following program requirements. Collaborate with providers, community health workers, case managers, and family members to support the patient’s healthcare journey and continuity of care. Use phone, email, and fax effectively to communicate with patients and healthcare partners, especially in contexts involving limited income and resources. Develop and maintain ongoing relationships with patients so that each participant knows their pharmacist and feels supported over time. Essential Skills Current, active pharmacist license in the State of Missouri. Ability to provide MTM-related services to Medicaid patients, including medication reviews, treatment planning, and adherence counseling. Strong telehealth and phone communication skills, with the ability to build rapport and trust remotely. Competence in assessing basic physical and mental health status, including use of DSM-based questionnaires and interpretation of clinical indicators such as glucose levels and blood pressure. Proficiency in coordinating with patient pharmacies, prescribers, caregivers, and other healthcare team members. Ability to perform medication reconciliation, address medication adherence issues, and deliver clear medication education. Strong documentation skills, including accurate recording of service details, time tracking in 15-minute increments, and treatment plans. Comfort using phone, email, and fax to communicate with patients and healthcare partners. Personable, compassionate, and patient demeanor, with a genuine passion for helping individuals who are less fortunate or have limited resources. Ability to manage an independent schedule and consistently complete approximately 8 hours of patient contact per day with timely documentation. Attention to detail and organizational skills to manage a high-volume patient panel over time. Additional Skills & Qualifications MTM experience is highly preferred. Experience in an ambulatory care setting or similar outpatient environment is a plus. Experience in an insurance or payer setting is beneficial. Prior enrollment with the state’s Medicaid program is a plus; if not already enrolled, willingness to complete enrollment (at no cost) prior to starting. Ability to write SOAP notes or similar clinical documentation is advantageous. Comfort working with patients who have chronic conditions and complex medication regimens. New pharmacy graduates are welcome and encouraged to apply. Strong interpersonal skills, with kindness, patience, and excellent phone presence. Commitment to providing accessible, personal, and ongoing care to Medicaid patients. Work Environment This is a fully remote pharmacist position that allows you to create your own work schedule within general pharmacy hours of approximately 6:00 a.m. to 7:00 p.m. You are responsible for scheduling about 8 hours of patient contact per day, with an additional 10–15 minutes of documentation after each call, and you may complete your patient load over the weekend if needed. All patients reside in Missouri and are enrolled in Medicaid, and services are delivered primarily via telehealth and phone, supported by email and fax communication. You will be part of a larger initiative designed to support 30,000–50,000 patients over the next few years, working within a cohort of compassionate pharmacists who value ongoing relationships with their patients. Job Type & Location This is a Contract position based out of Saint Louis, MO. Pay and Benefits The pay range for this position is $50.00 - $55.00/hr.
