What We’re Looking For Trillium Health Resources has a career opening for a Population Health Program Nurse to join our team! The Population Health Program Nurse is responsible for demonstrating clinical expertise for specific complex and/or rising risk member populations. The Population Health Program Nurse will work to identify members that are appropriate for population health programs utilizing data informatics along with providing interventions and education to promote health prevention and promotion. The Population Health Program Nurse will work as an integral part of the interdisciplinary team, ensuring excellence in member care to achieve optimal clinical outcomes with a focus on improving the health status of Trillium members. On a typical day, you might: Enroll identified Trillium members into appropriate population health programs. Support the build-out and management of future population health programs as indicated. Engage members by identifying mission/goals and objectives of the program along with assisting in completing a thorough clinical background. Work collaboratively with members of the multidisciplinary team to develop tools and interventions designed to achieve optimal patient outcomes. Coordinate and facilitate with Care Management staff timely implementation of assessments, care plans, and appropriate interventions for identified member populations following established policies and procedures. Must be able to travel within catchment as required. Preferred: Knowledge of Quality Management, Utilization Management, and discharge planning procedures as well as experience using data analytics for population health management. Certified Managed Care Nurse Certification or certification in care management.
