Practice Manager

We have a full-time vacancy for a Practice Manager for the Eastbourne District General Hospital Adult Social Care Hospital Discharge team. The team works alongside the Transfer of Care hub, to facilitate discharges for adults with complex needs on various discharge pathways. As a Practice Manager, you will play a key role in the day-to-day management of the team. The team undertake Care Act assessments and reviews of adults and carers who have been referred to your team and who are ready for discharge from hospital. You will be expected to work in a multidisciplinary way to support a person’s discharge and contribute to discussions that promote the person’s independence and best interests. As the manager of the team you will be responsible for oversight of the safeguarding enquiries held by the team, ensuring that we fulfil our duty under s42.2 of the Care Act 2014 by working with partner agencies to assess, and minimise safeguarding risk to the residents of East Sussex. We have excellent relationships with local partners and hospital trusts, and you will have the opportunity to work alongside other professionals to ensure good outcomes for the individual’s we are working with. Where necessary, we work with our local Integrated Commissioning Boards (ICB’s) to ensure people we serve get access to continuing care funding. We are looking for candidates who: Can work collectively with colleagues, both internally and externally Have experience of supervising Adult Social Care professionals or the potential to do so, for example development work in a health or social care setting Have the ability to make decisions when dealing with often constant and conflicting demands on time Please download the Job Description and Person Specification for further information.
