Building Name: UVMMC - In State Remote Worker Location Address: 111 Colchester Ave., Burlington Vermont Temporary Department: Community Health Strategy and Impact Full Time Standard Hours: 40 Biweekly Scheduled Hours: 80 Primary Shift: 8:00 AM - 4:30 PM Weekend Needs: As Scheduled Salary Range: Min $30.47 Mid $38.09 Max $45.71 Recruiter: Kate Davies Make a Difference Through Prevention The Culture, Experience and Community Health team at UVM Health is seeking a passionate and collaborative Prevention Coordinator to support community-wide efforts across the UVM Medical Center service area that promote health, strengthen protective factors, and prevent substance use and related behavioral health risks across the lifespan. In this role, you will design and implement programs that reduce risk factors and promote protective factors for youth and young adults, strengthe ning partnerships across Chittenden County. This position works closely alongside regional and local partners, including schools, healthcare providers, and community-based organizations. The Prevention Coordinator will facilitate upstream coalition and capacity-building activities within a comprehensive framework that complements downstream efforts of treatment, recovery, and harm reduction. This position supports priorities identified through the Community Health Needs Assessment and plays a key role in advancing evidence-based initiatives that advance heath equity. Please note: This is a hybrid role allowing for remote work and requiring an onsite presence at community events in the UVM Medical Center service area. The position is grant funded through 9/2030. WHAT YOU'LL DO Prevention Strategy & Program Implementation Design and implement evidence-based prevention initiatives focused on primary substance use prevention. Specifically, this position will focus on reducing the onset and progression of alcohol, nicotine , and cannabis use/misuse by youth and young adults. Participate in Prevention Network activities and convenings. Patriciate in regional Prevention Coordination Team, convened by United Way Northwest Vermont. Collaborate with similar Primary Prevention positions serving counties within Northwest and Central Vermont. Partner to strengthen prevention infrastructure and localized coordination and delivery of regional strategies. Align prevention activities with community -identified health priorities, local data, and t he Vermont Prevention Model. Community Engagement & Partnership Development Build and maintain strong relationships with schools, healthcare organizations, municipalities, government, nonprofits, and community leaders. Facilitate collaboration among diverse partners and community coalitions. Represent prevention priorities at community meetings, public forums, and coalition gatherings. Serve as a trusted resource for prevention information and best practices. Data, Planning & Evaluation Collect, analyze, and interpret community data to identify prevention opportunities. Use data and community feedback to inform planning and continuous improvement. Support program evaluation, outcome measurement, and reporting requirements. Track activities, deliverables, and grant-related outcomes. Education & Training Deliver prevention-focused education, trainings, workshops, and presentations. Support community awareness efforts that promote healthy behaviors and informed decision-making. Provide technical assistance and guidance to community partners implementing prevention activities. QUALIFICATIONS Required Bachelor's degree in Public Health, Prevention, Education, Social Sciences, or a related field, or equivalent relevant experience. Experience in prevention, public health, community health, education, or a related field. Experience coordinating programs, initiatives, projects, or community-based activities. Experience working with community collaboratives , coalitions, or multi-sector partnerships. Valid Vermont driver's license and ability to travel locally. Preferred Experience facilitating trainings, workshops, or educational presentations. Familiarity with evidence-based prevention strategies and public health frameworks. Experience using data to inform programs, planning, or community initiatives. Grant-funded program experience. WHY UVM Health?
