POSITION PURPOSE The Director, Adobe Provider Group (APG) Program is a strategic and operational leader responsible for overseeing field-based teams, including Nurse Practitioners (NP), Registered Nurses (RNs), Care Navigators and LVN Community Care Coordinators, and ensuring the delivery of integrated, whole-person care. This NP-licensed leader will be responsible for program development, clinical quality, team supervision, regulatory compliance, HIPAA compliance, and direct oversight of palliative care initiatives for medically complex and socially vulnerable populations. The Director ensures high-quality, equitable, and cost-effective care delivery in alignment with Adobe’s mission and contractual obligations. DUTIES & RESPONSIBILITIES Leadership, Hiring & Supervision Directly supervise a multidisciplinary team including, NPs, RNs, Care Navigators and LVNs. Hire, onboard, train, and mentor new staff; manage performance, implement corrective actions, and terminate when appropriate. Foster a culture of clinical excellence, compassion, accountability, and continuous improvement. Lead regular team meetings, one-on-one check-ins, and performance evaluations. Continually improve clinical care and oversight provided by the clinical team. Program & Operational Management Oversee the daily operations of APG’s field-based care programs across medical, behavioral, and social domains. Ensure all contractual Service Level Agreements (SLAs) and Key Performance Indicators (KPIs) are consistently met or exceeded. Implement performance dashboards and quality assurance protocols to monitor productivity, compliance, and outcomes. Lead the design and administration of a palliative care program for members with advanced or end-of-life conditions. Partner with leadership to align program goals, allocate resources, and adapt to evolving population health needs. Strategic & Clinical Partnerships Partner with the Senior Leadership to guide clinical quality and performance. Work closely with Senior Leadership to develop and execute strategies aimed at achieving performance, quality, and outcome targets. Participate in health plan-led patient rounds to discuss complex cases and improve member outcomes. Clinical Oversight Provide clinical leadership and consultation for complex case management scenarios and care planning. Guide the use of evidence-based protocols, motivational interviewing, trauma-informed care, and risk stratification. Support transitions of care and appropriate service referrals to improve member outcomes and reduce avoidable hospitalizations. Compliance & Quality Assurance Ensure strict adherence to HIPAA regulations and maintain the confidentiality and security of patient data. Monitor clinical documentation, ensure audit readiness, and implement corrective actions when compliance gaps are identified. Support internal and external audits and lead quality improvement initiatives across all levels of care coordination. Stakeholder & Community Engagement Act as primary liaison for payers, providers, and community partners. Represent APG at health-related events, coalitions, advisory boards, and business development engagements. Collaborate with public health agencies and community-based organizations to expand access to services. Other Perform other duties as assigned by management. SKILLS & QUALIFICATIONS Four (4+) years of nursing experience, preferably in population health, care coordination, or community-based programs. Two (2+) years of progressive leadership experience in population health, care coordination, or community-based programs. Demonstrated success in hiring, training, supervising, and managing performance of multidisciplinary teams. Deep understanding of SLAs, KPIs, Medi-Cal and Medicare Advantage programs, and palliative care operations. Strong knowledge and application of HIPAA and patient privacy standards. Excellent analytical, problem-solving, communication, and interpersonal skills. Proficient in EMR platforms (Salesforce preferred) and Microsoft Office Suite. Willingness and ability to travel frequently in-state and out-of-state as required. EDUCATION, LICENSES, & CERTIFICATIONS BSN required. MSN preferred. Active, unrestricted Nurse Practitioner (NP) license. Certification in Case Management (CCM) and Palliative Care (e.g., ACHPN) required within 2 years of position starting. Current BLS certification. Current and clear driver’s license. Proof of automobile insurance.
