A Quality Care Coordinator (QCC) contributes to AHF by improving health outcomes of patients receiving services throughout the AHF Healthcare System through compassionate proactive collaborative care. The QCC may be hybrid of both remote HCC locations and field work. QCC will demonstrate strong knowledge of Healthcare Effectiveness Data and Information Set (HEDIS), HIV AIDS Bureau Ryan White Standards.
A Quality Care Coordinator enhances the reputation of AHF by addressing preventive health measure outcomes of all payor source through improving and addressing HEDIS and HIV/AIDS Bureau measures, gaps in care, retention, and barriers to care. With radical collaboration with AHF departments and entities, with a solutions-oriented focus on telephonic interaction with patients.

AHF has a collaborative organizational structure where staff are accountable to multiple leaders.
The QCC will work as a part of a close-knit team to provide support to the Quality Care Management nursing team and provide outreach to patients, remove barriers and provide access so that patients are able to successfully complete preventative health measures regardless of payor source, follow up on transition of care and/or chronic care management appointments. The retention of patients is a priority for the QCC, the ability to connect with patients, provide compassionate care and empowerment is essential.
