Caring for Better Outcomes / Norfolk First Response
An exciting opportunity has arisen for Reablement Assistant Practitioners to join Caring for Better Outcomes (CFBO), a countywide, therapy-led service within Norfolk First Response. CFBO provides an additional reablement offer for people receiving support from a commissioned home-care provider agency.
Working jointly with home-care provider agencies, CFBO delivers frontline reablement for up to six weeks following a Pathway 1 hospital discharge. The service supports people to regain skills, build confidence and maximise independence at home.
You will work closely with Assistant Practitioners, Occupational Therapists, Operational Managers and a Practice Consultant Occupational Therapist to support timely progression through reablement. Following the reablement period, you will complete strengths-based Care Act assessments and arrange proportionate ongoing care and support where eligible needs remain.
Key responsibilities will include:
- Completing timely, proportionate and strengths-based Care Act assessments following reablement.
- Identifying eligible needs, agreeing person-centred outcomes and arranging sustainable support.
- Working with Assistant Practitioner Occupational Therapists to understand functional ability, progress and opportunities to maximise independence.
- Working with Operational Managers to progress cases, support timely decisions and avoid unnecessary delays.
- Building effective relationships with locality team managers and social care teams to support coordinated planning and appropriate handovers.
- Communicating effectively with people, families, provider agencies and partner professionals.
- Maintaining clear and accurate case records in line with statutory and organisational requirements.
- Managing and prioritising a countywide caseload according to need, risk and urgency.
You will be organised, able to prioritise in a fast-paced environment and committed to strengths-based, Home First practice. You will bring excellent communication, case-recording and partnership-working skills, alongside a commitment to high-quality social care practice. Regular supervision, training and peer support will help you develop in the role.
Benefits of working for Norfolk First Response:
- Competitive annual leave per year
- Excellent pension scheme
- Salaried post
- Extensive induction training and future development opportunities
- Norfolk Rewards Scheme (discount saving with various shops and restaurants)
- Paid business mileage
- Cycle to work scheme
- Part of Norfolk County Council
This is a highly visible, countywide-facing role. You will work across Norfolk, regularly engaging in person with people, families, provider agencies, CFBO colleagues, locality teams and partner professionals. The role follows a hybrid working model; however, applicants should expect frequent travel and a significant presence in community and office settings across the county to support effective relationships, coordinated decision-making and timely progression.
