You will assess and review their daily living needs, developing a personalised plan to assist them to regain the skills and confidence to remain independent in their home, and if ongoing services are required complete a Care Act Assessment. All training will be provided to ensure you are competent to deliver this critical role.
You will need to have experience of working with people and be able to communicate clearly and appropriately with members of the public, whilst exercising judgement and using your initiative to plan and prioritise your work. You will also need knowledge of current policy, practice and legislation within Health and Social Care.
We review applications regularly, so you need not wait until the closing date to apply.
Before you apply, we recommend reading the full Job Description and Person Specification to help you demonstrate you meet the criteria.
We currently have two posts available; both are full-time permanent contracts. One is based in Norwich, working in Norwich and surrounding areas. The other post is based in Dereham, working across South Norfolk. Please state in your application which location you are interested in or if you would like to be considered for both.
