Then we would love to hear from you! About the role: We have an exciting fixed-term contract (9 months) opportunity as a full time Referral Taker in our Hospital Discharge and Assessment Team. The Referral Taker is the first point of contact in the team; responsible for triaging information provided, liaising with partner agencies and taking comprehensive, holistic referrals. The role is an integral part of the team and will work closely with the team’s existing Referral Taker. You will work across both the Front Door and Discharge to Assess (D2A) pathways, collaborating closely also with Care Navigators. You will build and maintain strong working relationships with health partners, GCC teams, and the wider network of referral takers across localities. The post is based in Gloucester, and we offer flexible working with a mix of office and home working. The role will also support countywide teams and sites. About you: The successful candidate will have excellent communication skills and a willingness to work as a Team. They will have the ability to remain calm and focused on a busy environment while maintaining a positive attitude. The role needs a holistic approach and an ability to record accurate referrals and maintain service user records. You will learn to determine the appropriate level of urgency, ensuring they are directed to the right teams within HDAT to support timely and effective outcomes. There is also an element of signposting and directing to other agencies, ensuring to keep healthy working relationships in the process. You will have strong organisational skills with experience of responding flexibly to changing priorities. Experience of working in a customer facing environment is essential, as are excellent IT skills. Knowledge of the Liquid Logic (LAS) database and other health systems would be beneficial but full training will be given. Our values are at the heart of everything we do and how we treat each other.
