About Skyward Health Skyward Health is a New York State-designated Health Home dedicated to improving outcomes for individuals with complex medical, behavioral health, and social service needs. Through a robust network of Care Management Agencies (CMAs), Skyward Health coordinates person-centered care that connects members to essential healthcare, behavioral health, and community-based services. Skyward Health collaborates with providers, managed care organizations, hospitals, and community partners to improve care coordination, reduce barriers to care, and support healthier communities across the regions it serves. Job Summary The Regional Associate Director for the Long Island Region is a key leadership role reporting to the Director of Operations. Serving as the primary point of contact for an assigned network of Care Management Agencies (CMAs), this position is responsible for fostering strong partnerships, supporting operational excellence, and ensuring compliance with Health Home program requirements. The Regional Associate Director works closely with CMA leadership, Skyward Health senior management, and internal teams to support network performance, enhance service delivery, and promote growth across the Long Island region. This role provides guidance, operational oversight, and strategic support to all long Island CMAs while monitoring performance, satisfaction, quality outcomes, and regulatory compliance. The Regional Associate Director represents Skyward Health at regional and statewide forums, builds and maintains relationships with community stakeholders, promotes Health Home services, and supports network adequacy through referral development and CMA expansion efforts. This position also serves as an advocate for CMA partners and ensures that regional opportunities and challenges are communicated effectively to organizational leadership. Essential Responsibilities Regulatory Compliance & Program Oversight Remain current on New York State Department of Health (DOH), Office of Mental Health (OMH), and Health Home program requirements, policies, and initiatives. Participate in state webinars, workgroups, and subcommittees as directed. Assist in the development, implementation, and interpretation of internal and external policies and procedures. Ensure CMAs are informed of regulatory changes and supported in maintaining compliance. CMA Relationship Management & Performance Support Serve as the primary liaison for assigned Health Home Serving Adults CMAs. Provide timely support, guidance, and consultation to CMA leadership and staff. Facilitate regular regional meetings, forums, and collaborative discussions. Monitor network performance and utilize data to identify trends, opportunities, and areas requiring intervention. Partner with Quality and Operations teams to improve compliance, quality outcomes, and service delivery. Strategic Leadership & Operations Support organizational initiatives, projects, and operational goals established by the Director of Operations and senior leadership. Contribute to the development and implementation of internal processes and best practices. Prepare reports, presentations, and analyses to inform leadership decision-making. Ensure regional challenges, accomplishments, and opportunities are communicated effectively to senior leadership. Support continuous improvement efforts across the CMA network. Network Development & Referral Growth Identify and cultivate relationships with community-based organizations that serve eligible Health Home populations on Long Island. Conduct outreach, education, and presentations to increase awareness of Health Home services and generate referrals. Support the vetting, onboarding, and integration of new CMAs into the network. Monitor regional network adequacy and recommend strategies to address service gaps. Community Engagement & External Partnerships Develop and maintain relationships with healthcare providers, behavioral health organizations, managed care organizations (MCOs), hospitals, and community-based agencies across Long Island. Represent Skyward Health at regional, state, and community meetings, conferences, and forums. Promote collaboration that improves care coordination, access to services, and transitions of care for members. Serve as an ambassador for Skyward Health and its mission throughout the region. Member Services Oversight Address member inquiries, concerns, and complaints escalated to the Health Home. Collaborate with the Quality Team to coordinate care conferences and facilitate resolution of complex member cases. Support initiatives that improve member engagement, satisfaction, and outcomes. Additional Responsibilities Perform other duties and special projects as assigned. Education & Experience Bachelor's degree required; Master's degree in Social Work, Public Health, Healthcare Administration, Human Services, or a related field preferred. Significant Health Home experience may be considered in lieu of a master's degree. Minimum of three (3) years of experience working within the New York State Health Home Serving Adults program required; five (5) or more years strongly preferred. Knowledge of the Long Island Care Management Agency network and key community partners required. Experience with the Health Home Serving Children program is a plus. Clinical experience and a strong understanding of care management, quality assurance, compliance, and documentation standards required. Established relationships with healthcare organizations, community-based providers, and stakeholders throughout Nassau and Suffolk Counties preferred. Experience using Foothold Care Management, including billing, reporting, and care management functions, strongly preferred. Knowledge, Skills & Abilities Strong knowledge of New York State Health Home regulations, Medicaid programs, and care management best practices. Demonstrated ability to analyze data and use performance metrics to drive operational and quality improvements. Excellent communication, presentation, relationship-building, and organizational skills. Strong project management skills with the ability to manage multiple priorities and deadlines. Ability to build and maintain collaborative partnerships with internal and external stakeholders. Ability to work independently and effectively in a remote environment. Commitment to member-centered care, continuous quality improvement, and organizational excellence. Salary Range $83,000 – $88,000 annually , commensurate with experience and qualifications.
