Remote Clinical Pharmacist Medicaid MTM Case Manager

Join a compassionate, patient-focused team providing remote Medication Therapy Management (MTM) and clinical case management services to Medicaid patients across Missouri. Youll build ongoing relationships with patients, help them better understand and manage their medications, and collaborate with providers and community resources to improve health outcomes.

What Youll Do

• Provide MTM and comprehensive medication reviews via telehealth and phone
• Educate patients on medications, adherence, side effects, and chronic condition management
• Develop and monitor individualized medication treatment plans
• Identify and help resolve medication-related issues
• Coordinate with physicians, pharmacies, caregivers, case managers, and community health workers
• Assess patient needs using clinical indicators and basic physical and mental health screening tools
• Connect patients with healthcare and community resources when needed
• Maintain accurate, timely clinical documentation and continuity of care
• Build lasting relationships with patients who may have limited access to traditional healthcare

What Were Looking For

Required:

• Active Missouri pharmacist license
• Strong communication and patient education skills
• Compassionate, personable approach and the ability to build trust remotely
• Strong organization, attention to detail, and clinical documentation skills
• Ability to work independently in a remote environment
• MTM, ambulatory care, Medicaid, insurance, or payer experience
• Experience writing SOAP notes or similar clinical documentation
• Experience supporting patients with chronic conditions and complex medication regimens

Job Type & Location

This is a Contract position based out of Saint Louis, MO.

Pay and Benefits

The pay range for this position is $50.00 - $55.00/hr.

Individual compensation offered for this position within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors.
