RNLD Clinical Lead We're hiring a RNLD Clinical Lead Thornbury Community Services (part of Acacium Group) Permanent, Full Time Field based across North Yorkshire £49,000-£53,000 We are hiring a RNLD Clinical Lead to join Thornbury Community Services who are an 'outstanding' CQC-rated care provider that deliver complex care at home or in the community. In this role you will manage a caseload of packages to support individuals with learning disabilities, autism and mental health needs. In this role, you will: Undertake initial holistic nursing assessments and develop individualised care plans in partnership with the client, their family and the care team. Effectively communicate with the care team on the clinical condition or diagnosis and treatment or discharge plans of clients, ensuring accurate records are maintained. Identify and manage risks as part of care provision and lead the investigation and resolution of clinical incidents, ensuring timely and thorough documentation. Support the care team by delivering client-specific clinical training, mentorship and supervision by providing feedback, conducting performance reviews, and supporting professional development.
