Clinical Care & Resident Support Serve as the on-site clinical resource for residents, families, and community staff Assess residents for home health and private duty care needs Coordinate care with physicians, therapists, families, and interdisciplinary teams Perform skilled nursing visits, nursing assessments, and medication pours as appropriate Help ensure smooth transitions of care and continuity of services Relationship Building Develop strong relationships with residents, families, physicians, and community leadership Represent Health at Home with professionalism and compassion Participate in resident meetings, family conferences, and community events Provide wellness education, health screenings, and informational presentations Private Duty Clinical Support Provide guidance, training, and clinical oversight for private duty caregivers Assist with caregiver education and competency support Help ensure quality care and exceptional client experiences Compliance & Quality Maintain accurate, timely clinical documentation Ensure compliance with Medicare, state regulations, and agency policies Support quality improvement initiatives and patient safety standards Community Growth Help expand awareness of Health at Home's home health and private duty services Foster strong partnerships throughout the community Identify opportunities to improve resident care and service delivery What We're Looking For Required Qualifications Active Florida Registered Nurse (RN) license in good standing 1 to 3 years of nursing experience in home health, senior living, care management, or a related setting Excellent communication and relationship-building skills Passion for serving older adults Valid Florida driver's license and reliable transportation Preferred Qualifications Experience working in senior living communities Home health experience Community liaison or relationship management experience Strong organizational and time management skills The Ideal Candidate We're looking for someone who is: Compassionate and patient-focused Self-motivated and able to work independently A strong communicator and relationship builder Organized and detail-oriented Comfortable collaborating with physicians, families, and interdisciplinary teams Passionate about improving the lives of seniors Join Our Team At Health at Home, you'll have the opportunity to build meaningful relationships, improve patient outcomes, and make a lasting impact in the lives of older adults every single day.
