Senior Software Engineer
Astrana Health, Inc.Astrana Health builds the technology that lets physician groups deliver value-based care: the systems that enables physicians to deliver higher quality care and keeps its payers, and its partners in sync. Our engineers work on anything from eligibility, claims, and authorizations data, integrations with payer and provider systems, clinical integrations, provider-facing tools, and the data platform that ties them together.
The pace varies with the work. Some of it is fast iteration with real users. Some of it touches PHI, billing, or clinical decisions and has to be right the first time. We want engineers who can tell which kind of work is in front of them and adjust. We also expect engineers to work across the whole stack: database, backend, frontend, and the infrastructure that runs it.
As a Senior Engineer you own a product surface or a significant system end to end. We hand you problems rather than solutions. You take a fuzzy problem to a data model, an API, a UI, a rollout plan, and shipped software, and you bring the people around it with you: your PM, clinical and operations stakeholders, and the more junior engineers you mentor.
Our Values:
Our Values:
- Put Patients First
- Empower Entrepreneurial Provider and Care Teams
- Operate with Integrity & Excellence
- Be Innovative
- Work As One Team
What You'll Do
- Own the architecture, reliability, and technical direction for a product surface or system.
- Ship full-stack features end to end, from schema and API through to the React UI. Iterate quickly where the risk is low and be thorough where it is not.
- Decompose larger problems into work that several people can pick up in parallel, and adjust as you learn.
- Design data models that survive requirements nobody has heard yet, and integrations that stay correct when the systems on the other side misbehave.
- Work with the data directly. Answer questions about eligibility, claims, and quality data with SQL, notice when the numbers look wrong, and build the reporting and analytics that clinics and operations teams rely on.
- Run schema and data migrations safely: expand/contract, backfills, staged cutovers, and a rollback path you have actually thought through.
- Take systems from "works" to "runnable by someone else": meaningful logging, alerts, and runbooks.
- Build authorization, audit trails, and PHI minimization into your designs from the start.
- Work directly with product managers, practice managers, and clinicians. Come back from a twenty-minute conversation with a better spec than the one you were handed.
- Handle production incidents in your area without needing someone more senior to drive.
- Mentor junior and mid-level engineers, and review code in a way that teaches.
Qualifications
- 5+ years of professional software engineering experience, with a track record of delivering large, ambiguous features independently.
- Four-year undergraduate degree in Computer Science, Mathematics, Physics, or Engineering. Rare exceptions for outstanding experience.
- Full-stack experience. You are comfortable owning a feature from the database to the browser.
- Strong TypeScript, including on the backend.
- Strong SQL: you write and tune non-trivial queries, read query plans, and design schemas for data that will outlive the feature.
- Data and analytics savvy. You are comfortable exploring an unfamiliar dataset, validating it, and turning it into reports or metrics that non-engineers trust.
- Production experience with React.
- Docker and container-based development and deployment.
- Hands-on experience with at least one major public cloud (Azure, AWS, or GCP).
- Strong proficiency with AI development tools such as coding agents and assistants. You use them to move faster on the right work, review what they produce with the same rigor as any other code and know when a task is better done without them.
Preferred Qualifications
- Database modeling and migration frameworks such as Drizzle, Prisma, SQLAlchemy, or Alembic.
- Depth in Azure, AWS, or GCP beyond the basics, including cloud IAM, roles, and least-privilege access design.
- Python.
- Experience building LLM-driven applications: tool use, retrieval-augmented generation, evaluation, and guardrails.
- Healthcare data experience: eligibility, claims, clinical data, HL7/FHIR, or payer/provider integrations.
- Experience operating systems that handle PHI under HIPAA.
Environmental Job Requirements and Working Conditions
- Remote, US based position - with preference for candidates based in the greater Los Angeles area.
