About the Role
As a Supervisor on our Long Term Support and Services team, you’ll lead a dedicated unit that supports individuals in accessing services and long-term support solutions—most often referred to as waivers—through Pennsylvania’s Community HealthChoices program. You’ll guide a team of Service Coordinators as they help individuals navigate complex systems, coordinate care, and promote independence through person-centered planning. This role is ideal for someone who brings strong leadership, empathy, and an understanding of how to manage both people and processes in a high-accountability environment.
Starting Salary: Based on experience and qualifications
Core Responsibilities
- Ensure that service plans are developed and implemented
- Ensure monitoring and follow-up are conducted appropriately
- Verify waiver eligibility and Medical Assistance requirements
- Support employees in meeting documentation and billing standards
- Monitor unit productivity and provide coaching as needed
- Oversee the incident management and reporting process
- Ensure timely and accurate completion of assessments and planning documents
- Provide direct supervision and caseload support when necessary
- Complete required trainings and prepare program reports
- Maintain cooperative and professional relationships with colleagues and stakeholders
What You Bring
- RN, or a Pennsylvania-licensed social worker or licensed mental health professional
- At least three (3) years of relevant experience
- Supervisory, leadership, or management experience preferred
- Ability to manage time effectively and respond to changing priorities
- A valid Pennsylvania driver’s license, automobile, and required clearances

Location & Schedule
This position is based in the Berks County area, with remote work and community-based service. Office hours are Monday through Friday, 8:30 AM to 4:30 PM.
