This position can be based out of either our Steamboat or Craig office.


The SHIE Outreach and Operations Coordinator supports The Health Partnership’s Social Health Information Exchange (SHIE) initiative by engaging community members and partners, promoting SHIE Hub resources, and helping organizations use shared referral tools to connect people with available services. This role combines relationship building, partner education and onboarding, and hands-on platform support. Working with the SHIE team, project manager, and participating organizations, the coordinator helps maintain accurate resource information, resolve user questions, and strengthen care coordination across Northwest Colorado. This position dedicates 100% of its time to the SHIE project.

Supervision Received: The SHIE Outreach and Operations Coordinator is supervised by the Community Impact Director.


Supervision Exercised: None. This position does not supervise staff.


Key Accountabilities:

Provide platform and operational support

  • Learn to use the referral software and help THP staff and partners feel comfortable using it for care coordination and referrals.
  • Support partners in following established data-sharing practices, coordinating questions with the SHIE team and reinforcing approved procedures for sharing information.
  • Use Excel spreadsheets to organize information, map data fields, review listings, and identify missing or inconsistent information.
  • Help keep resource information accurate and aligned between the referral platform and the WordPress resource directory.
  • Investigate user questions and problems, document what is happening, and work with the project manager or vendor to resolve issues.
  • Develop and maintain user instructions, track recurring issues, and suggest ways to make the system easier to use.
  • Support routine checks of platform activity, information accuracy, and user access; escalate concerns to the appropriate team member or vendor.

Build and strengthen equitable, community-centered partnerships to support access to care through SHIE

  • Develop and maintain relationships with clinical and behavioral health providers, community-based organizations, and other partners across Northwest Colorado.
  • Promote awareness and use of SHIE Hub resources by engaging community members and organizations and demonstrating how SHIE supports coordinated referrals and connections to services.
  • Consider barriers to access, rural community needs, and partner capacity when supporting outreach, onboarding, training, and referral workflows.
  • Gather and respond to partner and community feedback to identify gaps in resource information, referral processes, or system usability, and recommend improvements that advance equitable access to care.
  • Conduct ongoing partner outreach and follow-up to understand successes and challenges, strengthen engagement, and connect partners with appropriate support and resources.

Serve as an educational liaison to support SHIE understanding, engagement, and adoption

  • Serve as a liaison between the SHIE initiative and participating providers, community-based organizations, and community stakeholders.
  • Help partners prepare to join by gathering information about their services, staff, and referral processes.
  • Support setup of new partner organizations, users, referral workflows, and resource listings.
  • Coordinate partner training, testing, and follow-up so new partners can begin using the system with confidence.
  • Explain technical steps and referral workflows in plain language, adapting instruction to different levels of technology experience.

Collaborate with the SHIE team and managers to meet program deliverables and support evaluation

  • Coordinate with the SHIE team, project manager, and managers to track assigned activities, deadlines, and program deliverables.
  • Maintain documentation of partner onboarding, training, follow-up, and system issues to support program reporting.
  • Collect feedback from community members and partner organizations to inform ongoing project improvements.
  • Support program evaluations by gathering requested program information and help implement improvements identified by the team.


General Requirements & Qualifications:

  • Commitment to inclusiveness, social justice, health equity, and reducing barriers to care.
  • Proficient with Microsoft Office/365 and comfortable learning and navigating new technology and software systems.
  • Comfortable using Excel to organize information, review data, and identify inconsistencies; willing to learn new systems.
  • Enjoys learning software, working through workflows, and solving practical problems.
  • Able to explain technical steps in plain language to people with different levels of technology experience.
  • Organized and patient, with the ability to balance hands-on system work, partner follow-up, and relationship building in rural communities.
  • Able to handle sensitive information carefully and follow THP’s confidentiality, privacy, and information security procedures.
  • Must possess a valid Colorado driver’s license, maintain reliable transportation, and be willing to travel throughout the region, including during inclement weather.
  • Must provide proof of a valid driver’s license and adequate insurance coverage totaling at least $300,000 per occurrence.
  • Performs all other duties as assigned.

Working Conditions:

  • This role includes a combination of office-based, community-based, and remote work.
  • Regular travel throughout Routt and Moffat counties is required.
  • Flexible scheduling may be available based on program and client needs.
  • Work location for administrative activity is in an accessible office environment.
  • Daily activity is 80% sitting or standing with extended periods of typing at a keyboard, 20% walking with occasional stooping, bending, reaching, twisting.
  • Office equipment would include a phone, computer, printer, and copier on a daily basis.
  • Independent travel throughout the region, including during inclement weather.

Compensation:

This is a salaried, non-exempt position, compensated on a salary basis and eligible for overtime pay in accordance with applicable Colorado and federal wage and hour laws. Accurate recording of all hours worked is required.

$60,000-$63,000 annually
