Supervisor Care Management RN- Beacon Hill, WA
Schedule- Full Time/ Hybrid
Provides leadership and supervision of the day-to-day clinical and administrative operations of the care management team. Supervises Care Management (CM) staff to include nursing, social work, patient care coordinators and other non-clinical staff to achieve the goals for the organization; collaborates directly with inpatient and ambulatory clinicians and care teams, as well PacMed and PCN leaders to support cost-effective high-quality medical services.

Required Qualifications:
- Coursework/Training Nursing (must be RN) And
- Upon hire: Washington Registered Nurse License
- 5 years Clinical nursing, including acute care hospital and/or ambulatory clinic experience.
Preferred Qualifications:
- Bachelor's Degree Nursing
- 3 years Case Management experience including care management, care coordination, discharge planning, or population health.
- 2 years Progressive supervisory experience.
