Employs a variety of strategies, approaches and techniques to manage a member's health issues telephonically. Identifies and resolves barriers that hinder effective care. Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through use of assessments, data, conversations with member, and active care planning. Understands own work area professional concepts/standards, regulations, strategies and operating standards. Makes decisions regarding own work approach/priorities and follows direction. Work is managed and often guided by precedent and/or documented procedures/regulations/professional standards with some interpretation. Follows established guidelines/procedures. Use your skills to make an impact Required Qualifications Bachelor's degree in a health-related field, active Illinois LPN licensure, or the ability to obtain IL LPN license. 2 or more years of case management, medical case management, care coordination, or healthcare support experience. Comprehensive knowledge of Microsoft Office applications including Word, Excel, and Outlook. Previous experience with electronic case note documentation and experienced with documenting in multiple computer applications/systems. Experience with health promotion, coaching and wellness. Knowledge of community health and social service agencies and additional community resources. Preferred Qualifications Bi-Lingual (English/Spanish) Certified Case Manager (CCM) Master's Degree Managed care experience Prior experience with Medicare & Medicaid recipients Additional Information Workstyle: remote, work from home. Typical Work days/hours: Operating hours are Monday - Friday 8:00 AM - 7:00 PM CST but core hours for this position are primarily Monday – Friday, 8:00 AM – 5:00 PM CST. May need to be flexible with work schedule based on business needs. Work at Home Requirements: To ensure Home or Hybrid Home/Office employees’ ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office employees must meet the following criteria: At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is required; wireless, wired cable or DSL connection is suggested. In certain roles, the minimum recommended internet speed required by Humana may not be sufficient for business needs. Humana reserves the right to require associates to upgrade their internet service if necessary. Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information. Travel: While this is a remote position, occasional travel to Humana's offices for training or meetings may be required. Scheduled Weekly Hours 40 Pay Range The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting.
