Job Description - Transfer Cednter RN- CareComm (260003I2) Job Description Transfer Cednter RN- CareComm - ( 260003I2 ) Description Under the direction of the CareComm Operations Manager and in accordance with established policies, procedures, and regulatory requirements, the Transfer Center Registered Nurse (RN) evaluates the appropriateness of patient admissions and transfers to Tampa General Hospital based on clinical presentation and acuity. The Transfer Center RN gathers and analyzes clinical information, triages incoming calls, and coordinates with appropriate specialty services to facilitate timely and safe patient transfers and direct admissions.As a patient advocate, the Transfer Center RN expedites admissions following acceptance and ensures continuity of care throughout the transfer process. Given Tampa General Hospital's role as the region's only Level I Trauma and Burn Center and one of the few facilities with a Level III NICU, the Transfer Center RN must demonstrate advanced clinical judgment and the ability to triage and manage critically ill and complex patient transfers while maintaining compliance with EMTALA, state statutes, and organizational standards. What you will do Assesses clinical information to determine appropriateness for admission or transfer to Tampa General Hospital based on medical presentation and acuity. Triage all transfer and direct admission calls with appropriate urgency, prioritizing critically ill and time‑sensitive cases. Collects, reviews, and documents clinical information, including diagnoses, isolation needs, transportation requirements, and special care considerations. Connects referring providers with appropriate specialty teams and ensures all involved services are notified and aligned with the patient’s care needs. Acts as a patient advocate by expediting accepted admissions and transfers to meet departmental and organizational throughput targets. Screens patients for sepsis, confirms isolation requirements, and ensures compliance with EMTALA and applicable state regulations. Accurately documents the reason for transfer, physician communications, and required clinical updates per shift in the electronic health record. Coordinates and prioritizes multiple transfer and admission requests simultaneously, adjusting workflow as clinical urgency changes. Works with internal and external resources to arrange appropriate outpatient follow‑up or alternate care when patients are not suitable for admission or transfer. Serves as a clinical resource to CareComm patient flow teams, identifies opportunities for quality and safety improvements, and promotes a positive, solution‑focused work enviro Qualifications Bachelor’s Degree Nursing Registered Nurse Graduate of accredited School of Nursing; licensure to practice as a Registered Nurse by the State of Florida. Two (2) years' nursing practice.
