What You'll Do

As a Transportation Navigator, you'll help individuals and families overcome transportation barriers that impact their access to healthcare, employment, education, childcare, and other essential services. • Coordinate transportation solutions for healthcare, employment, education, childcare, and other essential needs.
  • Assist participants with resource navigation, referrals, and connections to community programs and supports.
  • Screen for transportation stability needs, including vehicle-related barriers and emergency transportation assistance.
  • Maintain regular contact with participants through phone, virtual, and in-person meetings as appropriate.
  • Build relationships with transportation providers, community partners, healthcare organizations, and other service agencies.
  • Maintain accurate participant records, service documentation, and outcomes in agency systems.
  • Support outreach activities, regional transportation initiatives, and community partnership efforts.
  • Participate in required training and ongoing professional development.

Most importantly, you'll serve as a trusted resource, helping individuals and families access reliable transportation solutions that improve their health, stability, and quality of life.

What We'll Love About You:

  • High school diploma or GED required. Associate Degree in Social Work, Human Services, Sociology, Psychology, Public Health, Community Health, or a related field preferred.
  • Two years of experience working with vulnerable populations, families experiencing poverty, case management, mentoring, transportation coordination, community health navigation, or a related human services field preferred.
  • Experience with participant goal planning, resource navigation, community partnerships, and case documentation preferred.
  • Strong communication, organizational, and relationship-building skills, with the ability to work independently and as part of a team.
  • Proficiency with Microsoft Office, databases, electronic documentation systems, and participant tracking software.
  • Valid Michigan driver's license and reliable transportation required.

Most importantly, you're passionate about helping families overcome barriers, build self-sufficiency, and achieve their goals through compassionate support, mentoring, and advocacy.
