At R1, we are looking for a strategic and operational leader who can drive enterprise-level revenue cycle performance while serving as a trusted advisor to one of our largest health system partners. This role is for a leader who anticipates challenges before they surface, brings solutions instead of problems, leverages data to influence decisions, and thrives in a highly visible client-facing environment.


As Vice President, Self-Pay Performance & Client Strategy, you will provide strategic performance leadership and dotted-line accountability across Self-Pay, Early Out, Patient Financial Experience, Charity, and Bad Debt operations while partnering directly with senior client executives to improve financial performance, advance innovation, and deliver measurable value. Success requires executive presence, operational rigor, strategic creativity, and the ability to navigate complex stakeholder relationships with confidence and professionalism.

Every day in this role, you will:

  • Own the analytical assessment of financial, operational, and client performance across a large-scale Self-Pay portfolio.
  • Serve as a strategic advisor and day-to-day performance partner to operational leaders, the client partner, and client stakeholders.
  • Lead revenue cycle transformation initiatives that focus on patient cash acceleration, bad debt reduction, charity optimization, automation, and operational efficiency.
  • Drive continuous improvement efforts that deliver measurable financial value and cost savings.
  • Translate data into actionable insights and executive-level recommendations through hands-on SQL and Snowflake analysis.
  • Lead and influence cross-functional operational, analytical, technical, and client-facing teams.
  • Develop strategies that improve patient financial outcomes while reducing administrative burden.
  • Develop an integrated patient engagement strategy across texting, dialing, email, and other channels that support the broader patient collections strategy.
  • Build credibility with senior executives through proactive communication, operational transparency, and consistent execution.
  • Hold vendors accountable for performance, service levels, and results, and clearly communicate vendor performance to client leadership.
  • Identify risks, trends, opportunities, and performance gaps before they become escalations.
  • Partner closely with operational leaders through dotted-line accountability to establish priorities, monitor execution, and resolve performance gaps.
  • Serve as a conduit to Patient Access, Payer Yield, and other upstream teams to improve point-of-service collections, demographic and address accuracy, statement readiness, and patient-responsibility balance accuracy.
  • Partner across upstream payer yield, billing, and collections functions to improve the accuracy and flow of patient responsibility across both hospital billing (HB) and professional billing (PB).
  • Utilize Epic, Snowflake, advanced SQL, reporting platforms, and operational analytics to identify opportunities and drive performance improvement.
  • Influence executive decisions through facts, insights, business cases, and measurable outcomes.

To be successful in this role, you must have:

  • Bachelor's degree from an accredited college or university.
  • 10+ years of progressive Revenue Cycle leadership experience.
  • Experience leading Self-Pay, Patient Financial Services, Revenue Cycle Operations, Bad Debt, Early Out, Patient Cash, or related functions.
  • Deep working knowledge of self-pay accounts receivable across both hospital and physician practice environments, including the distinct HB and PB workflows and their convergence within an Epic Single Billing Office (SBO) model.
  • Demonstrated ability to influence large-scale operational teams and drive accountability without direct reporting authority.
  • Demonstrated success managing executive-level client relationships.
  • Strong financial acumen with experience owning operational performance, budgets, forecasts, and KPIs.
  • Advanced analytical skills, including deep hands-on experience writing complex SQL queries, working in Snowflake, and using large datasets to identify trends, forecast outcomes, and drive decisions.
  • Experience with Epic and enterprise reporting tools.
  • Knowledge of state guidelines and collection regulations, with the ability to incorporate requirements into the self-pay approach and model operational and financial impacts.
  • Proven ability to lead transformation, change management, and process improvement initiatives.
  • Hands-on knowledge of Epic builds, configurations, and work queues, with experience partnering with IS and vendors through technology implementations, vendor changes, and large-scale SBO transformation.

Preferred Qualifications:

  • Experience supporting large integrated health systems.
  • Prior experience leading an Epic Single Billing Office (SBO) team responsible for self-pay operations spanning both HB and PB.
  • Consulting or client-partner leadership experience.
  • Track record of reducing bad debt, improving patient cash collections, and optimizing patient financial experience.

For this US-based position, the base pay range is $179,593.00 - $280,035.56 per year . Individual pay is determined by role, level, location, job-related skills, experience, and relevant education or training.

This job is eligible to participate in our annual bonus plan at a target of 30.00%
