- Maintain Compliant Whole-Person Documentation: Complete detailed, timely clinical documentation (DAP format) capturing behavioral health interventions, medical coordination, and recovery progress to support program quality standards under both bundled PMPM and encounter-based models.
- Drive Multimodal Recovery Engagement: Utilize multi-platform technology (video, secure messaging, digital recovery content, and mobile tools) to maintain persistent, high-frequency outreach and re-engage members experiencing ambivalence or relapse risks.
- Manage Panel Registry & Infrastructure: Actively participate in interdisciplinary clinical huddles, maintain registry data integrity, and track clinical touchpoints (up to 24 structured encounters) to optimize panel outcomes and ensure seamless care coordination.
- Coordinate Medical & Community Wrap-Around Supports: Partner with external MAT/MOUD providers, medical teams, and community recovery organizations to address members' holistic medical, physical health, and health-related social needs.
- Collaborate with the member, family, and healthcare providers to develop an individualized plan of care.
