Description The Bilingual Community-Based Social Worker provides culturally responsive services and leads a collaborative planning process for youth and families who are referred through Marin Special Education Plans. Services will be provided almost exclusively in the community: in families' homes, foster homes, schools, other community locations, etc. for youth who are at-risk of being placed in out of home care or are stepping down from a higher level of care. The primary role is to ensure a sustainable network of unpaid supports (i.e. extended family, mentors, teachers, coaches) are working together toward stability for the youth and family. About North Bay School Partnerships The North Bay School Partnership (NBSP) program supports students who are experiencing barriers to successful academic functioning. NBSP utilizes strategies from the High-Fidelity Wraparound model to assist students and their families develop a care plan that leverages their strengths and mitigates the impact of barriers to education. Services are primarily provided at home, in the community, or on school campus, as needed. Average service duration is approximately four to six months, and students are referred through the school district or their individual education plan team. Responsibilities: Function as the team clinical leader for respective caseload, and provide direct services that are culturally responsive to youth and caregivers on a weekly basis to further treatment goals. Provide psychoeducation, caregiver coaching, and coordinate care between providers and school district partners, etc. Identify Family Finding and engagement needs and develop Youth and Family teams made up of natural supports for the purpose of treatment planning and sustainable lifelong support. Facilitate Child and Family Team meetings (CFTMs) monthly or as needed that maintain compliance to the Wraparound Model. Cultivate and maintain engagement and alignment with school district partners, community members, county partners, and other MH providers. Lead case planning and consultation with team members on clinical issues, as well as casework issues, as appropriate. Complete mental health notes, clinical documentation (i.e. Assessments & treatment plans), safety plans, incident reports, and other documentation accurately and in a timely manner. Facilitate multi-disciplinary or Child and Family Team meetings (CFTMs) at least monthly. Participate in the 24-hour on-call support line system on a shared rotation. Participate in individual and/or group supervision weekly as well as other required internal staff meetings or trainings. Qualifications Required Master’s degree in social work/psychology/counseling. Registered with the California Board of Behavioral Sciences. Bilingual in English/Spanish Experience working with children in out-of-home care preferred. At least 21 years of age. TB test clearance, fingerprinting clearance, and any other state/federal licensing or certification requirements. Valid driver's license, clean driving record, and insurability through Seneca Family of Agencies’ insurance policy. Skills of an Ideal Candidate Ability to work as part of a multidisciplinary team. Ability to work with a diverse population. Good organizational and writing skills. Schedule: Monday - Friday, 9:00 AM - 5:30 PM Benefits: Starting at $82,157 - $96,157 per year. Actual salary dependent on creditable experience above the minimum qualifications for the role. Additional $4000 salary increase for BBS licensure. 5 weeks of Paid Time off and 11 Paid Holidays.
