Role: Case Manager (Child Youth and Family)

Location: 92 Dalton Street, Orange, NSW

Employment Type: Permanent Full Time

Hours of Work: Monday to Friday, 8.30am – 5pm

Remuneration: From $95,970 per year + Super + Tool of Trade Vehicle

As a Case Manager, you will play a pivotal role in providing holistic support to young people in need. Your primary responsibility will be to work closely with young people, who are experiencing a range of emotional, behavioural, and psychological difficulties.

Through your expertise in therapeutic intervention and case management, you will help guide them towards improved mental well-being, self-esteem, and life skills.

The successful candidate will also conduct weekly home visits across the Western region (Orange, Dubbo, and Bathurst), as well as participate in an on-call roster 2 days per fortnight 5pm to 9am the following morning (subject to change based on business requirements).

Key Responsibilities

  • Develop and review case plans in collaboration with relevant Care Team members, addressing the individual needs of children and young people in LWB's care.
  • Address the impacts of abuse, neglect, separation from families and complex trauma on children and young people in out of home care.
  • Engage in reflective practice, assessment, development, implementation, monitoring, and case plan reviews to ensure young people's needs are met.
  • Maintain high-quality, up-to-date record keeping for each child or young person.
  • Support children and young people in developing and maintaining positive relationships with family, community members and connection to culture.
  • Provide ongoing support and supervision to Carers to assist them implement strategies outlined in relevant plans through home visits and phone support.

Skills & Experience

  • Relevant bachelor’s degree, or a relevant diploma and be working towards a degree. (A qualification is desirable but not essential for Aboriginal and/or Torres Strait Islander applicants with relevant experience and a willingness to undertake further training)
  • Previous case management experience within an Intensive Therapeutic Care or Out of Home Care (OOHC) setting
  • Experience supporting Children & Young People who have experienced backgrounds of trauma, abuse and/or neglect.
  • Understanding of current casework practices for children and young people
  • Demonstrated skills in working with individuals, families, or groups using a strengths-based and culturally appropriate approach
  • A current driver’s licence.

The successful candidate will be required to clear probity checks including National Criminal History Record Check; NSW Working with Children Check and a Residential Register Worker Clearance.
