Job Overview

We are seeking an experienced clinical practitioner with a high level of skill and theoretical knowledge. The purpose of this role is to provide specialist nursing care, advice, guidance and leadership, and to take responsibility for a specialist nursing service.

You will assess, plan, implement and evaluate evidenced-based nursing care and provide expert advice and clinical practice.

You will work autonomously and manage your own workload as a lead specialist in your area, provide specialist education and training to staff.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Main duties of the job

As a Clinical Nurse Specialist, you will practice autonomously, demonstrating expert problem solving and clinical decision-making skills while demonstrating accountability and responsibility (NES).

Assessing, managing, delivering and co-ordinating the plan of care for patients utilising the effective and efficient use of resources

You will undertake investigations driven by protocol and within your competence. Interpret results and act accordingly.

Providing specialist, expert clinical knowledge to clinical colleagues, patients and carers/relatives.

Assessing, diagnosing, planning, implementing and directing care / interventions and evaluating effectiveness (NES).

You will delegate the delivery of care where appropriate in line with NHS Wales’ delegation framework and NMC Code of Professional Practice (2018).

Undertaking clinical sessions in a variety of clinical areas across HDUHB.

Following the necessary training and competencies, the post holder will be required to perform highly specialised physical and clinical skills where accuracy is of paramount importance in accordance with departmental policies.

You will undertake home visits as necessary.

Undertaking non-medical prescribing – following successful completion of the relevant nonmedical prescribing course.

You will undertake patient specific education with patients and relatives

Working for our organisation

Hywel Dda University Health Board plans and provides NHS healthcare services for people living in Carmarthenshire, Ceredigion, Pembrokeshire, and bordering counties.

We have over 13,000 staff and together we provide primary, community, in-hospital, mental health and learning disabilities services.

We work in partnership with the three local authorities, as well as public, private and third sector colleagues, including our valued team of volunteers.

Our services are provided in:

  • Four main hospitals: Bronglais Hospital in Aberystwyth; Glangwili Hospital in Carmarthen; Prince Philip Hospital in Llanelli; and Withybush Hospital in Haverfordwest
  • Five community hospitals: Amman Valley and Llandovery hospitals in Carmarthenshire; Tregaron Hospital in Ceredigion; and Tenby and South Pembrokeshire hospitals in Pembrokeshire
  • Two integrated care centres: Aberaeron and Cardigan in Ceredigion, and several other community settings
  • 47 general practices (six of which are health board managed practices); dental practices (including four orthodontic); 99 community pharmacies; 43 general ophthalmic practices; and 8 ophthalmic domiciliary providers
  • Numerous mental health and learning disabilities services
