Job Overview
We are seeking a motivated and compassionate Enhanced Clinical Practitioner (ECP) to deliver high-quality, evidence-based care to patients with undiagnosed, evolving or complex health needs across a range of community and urgent care settings. Working within an agreed scope of practice and supported by senior clinicians, the post holder will undertake clinical assessment, examination, decision-making and management to provide safe, effective and person-centred care.
The successful candidate will work autonomously whilst recognising the limits of their competence, escalating appropriately when required and collaborating closely with multidisciplinary teams to ensure patients receive the right care, in the right place, at the right time. A key focus of the role is supporting timely intervention, admission avoidance and care closer to home.
The post holder will contribute to clinical triage, assessment, treatment planning, care coordination and service delivery across health facilities, patients' homes and virtual environments. They will play an active role in clinical governance, quality improvement, education and service development while progressing through the Health Board's ECP competency framework.
Main duties of the job
The post holder will undertake comprehensive clinical assessments, including history taking, physical examination, clinical reasoning and decision-making, to develop and implement appropriate treatment and management plans.
Working autonomously, with support from senior clinicians when required, to assess, triage and manage patients across a variety of settings, including healthcare facilities, patients' homes and virtual consultation environments. The post holder will ensure patients receive timely access to appropriate care, treatment or onward referral, while recognising the limits of their competence and escalating concerns as necessary.
A key responsibility is promoting care closer to home and ensuring patients receive the right care in the most appropriate setting.
The post holder will contribute to clinical governance, quality improvement, audit, service evaluation and service development activities, supporting the delivery of safe and effective care. They will also promote health education and self-management, participate in teaching and mentorship activities, and maintain a portfolio of evidence to demonstrate progression through the Health Board's Enhanced Clinical Practitioner competency framework.
The role demands excellent communication and teamwork skills, flexibility, professional accountability and a commitment to ongoing professional development to ensure the delivery of person-centred care across urgent, acute, community and integrated care services.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
