Location: LIFE Geisinger Wilkes-Barre Shift: Days (United States of America) Scheduled Weekly Hours: 40 Worker Type: Regular Exemption Status: Yes Job Summary: LIFE Geisinger (Living Independently for Elders) is seeking an experienced geriatrician, family medicine or internal medicine physician for our LIFE Geisinger South Wilkes Barre, Program of All-Inclusive Care for the Elderly (PACE) program, in Wilkes Barre, PA Job Duties: LIFE Geisinger provides additional independence for older individuals and their caregivers. We can help them stay in their homes while taking advantage of comprehensive daily living and health services. It's the best of both worlds. LIFE Geisinger offers a unique, team-based model of care for older adults with complex medical needs. Physicians collaborate with an Advanced Practitioner, pharmacists, therapists, social workers, care coordinators and families to deliver comprehensive, relationship-centered care. Highlights: Typical schedule of 12-16 patients per day 30-minute appointment times for thoughtful, comprehensive care Mix of chronic disease management, acute visits, post-hospital follow-ups, and annual assessments Collaborative interdisciplinary care team providing extensive clinical support Occasional home visits and monthly skilled nursing facility visits One week of call per month with nurse triage managing first-call responsibilities At LIFE Geisinger, physicians enjoy a slower-paced, mission-driven practice environment focused on helping older adults remain healthy, independent, and thriving in their homes and communities.
