Home Health Physical Therapist / PT (Full-Time)

Role: Physical Therapist in a home health setting, responsible for evaluating patients, developing individualized treatment plans, and implementing therapeutic exercises to improve mobility, strength, and pain relief.

Key Responsibilities

• Assess patients' physical conditions and create tailored treatment strategies.
• Implement therapeutic techniques and exercises to enhance patient outcomes.
• Educate patients and caregivers on exercises, safety, and self-care.
• Collaborate with interdisciplinary teams to deliver holistic, patient-centered care.

Qualifications & Skills

• Active Physical Therapist license in the practicing state.
• Master’s or Doctorate degree in Physical Therapy.
• At least 1 year of experience, with home health experience preferred.
• Current BLS and CPR certification.
• Valid driver’s license and reliable insured vehicle.
