Cunningham Children’s Home is seeking a compassionate, organized, and collaborative Integrated Care Coordinator to join our Community Services team. This position plays a key role in supporting youth and families in Champaign County wh o are at risk of becoming involved in the juvenile justice system. The Integrated Care Coordinator coordinates services through the Families Stronger Together (FST) and Juvenile Redeploy Illinois (JRI) programs, working closely with youth, families, schools, court services, attorneys, community providers, and other partners to promote stability, strengthen families, and help youth remain safely connected to their communities. What You'll Do: Coordinate referrals, services, and individualized plans for youth and families. Facilitate family and team meetings and coordinate communication among service providers and community partners. Use a strengths-based, trauma-informed, youth-driven, and family-guided approach. Monitor progress, support crisis and intervention planning, and promote stability for youth and families. Participate in court-related activities and collaborate with juvenile justice partners. Maintain accurate case records, reports, assessments, and grant-related documentation. Participate in community meetings, training, quality improvement, and program development. Qualifications: Bachelor’s degree in human services or a related field such as social work, counseling/clinical psychology, or juvenile/criminal justice; Master’s degree preferred. Three years of related experience working with youth and families; experience in child welfare, behavioral health, or juvenile justice is preferred, particularly experience with Champaign County juvenile justice services preferred. Strong organizational, communication, documentation, and relationship-building skills. Ability to work independently, manage multiple priorities, meet deadlines, and collaborate effectively with youth, families, and community partners. Valid Illinois driver’s license, current auto insurance, and reliable access to a vehicle for work-related travel. The core schedule is Monday–Friday, 8:30 a.m.–5:00 p.m., with flexibility required to meet the needs of youth and families. This position may require up to three evenings per week and occasional weekends.
