Educate patients and families on adaptive techniques, home exercises, and equipment use. Collaborate with physicians, nurses, and other rehabilitation professionals to coordinate care. Document progress, adjust treatment plans, and communicate updates to the care team. Ensure compliance with clinical standards, safety protocols, and ethical practice guidelines. Participate in team meetings, patient rounds, and discharge planning discussions.
